Section 7 – OH&S Reporting and Investigation Version: 02 Page: 1 of 1 Issue Date: 12 January 1999 Authorised by: Marjorie Dickenson OH&S REPORTING AND INVESTIGATION All work-related injuries at SimuLab MUST be reported on the SimuLab workplace injury/illness/incident report form and reported to the Human Resources Manager within 48 hours of occurrence. Any lost time injury MUST be reported to the Human Resources Manager within 24 hours of occurrence. 7.1. Injury/Illness/Incident Reporting 7.1.1. Incidents These are events that might have resulted in injury, but did not. A good example of this is a chemical spill or fire, which resulted in no injury and minor damage. In general these point to the likely occurrence of an accident in the near future unless appropriate risk assessment and control measures are implemented as soon as possible. All incidents should be reported using the SimuLab form 05-100, Injury/Illness/Incident Report. Additionally all incidents need to be investigated so that corrective action can be implemented to reduce the possibility of a reoccurrence or injury. Investigations should be undertaken according to the SimuLab Risk Management Policy and the SimuLab Accident Investigation Guidelines. Accident investigations are completed using the SimuLab form 05101, Accident – Incident Investigation, then forwarded to the Human Resources Manager. Ensure that: Any hazards identified in the investigation process are logged in the Risk Register. Information about the hazard is communicated to other sections who may face similar hazards. 7.1.2. Injury and Illness Reports Responses to injury and illness depend upon severity. In general the following process should be followed: Request the assistance of the First Aid Officer. If a First Aid Officer is unavailable for a serious injury, call the ambulance or qualified medical personnel. Complete a SimuLab form 05-100, Injury/Illness/Incident Report. If the injured person is a SimuLab employee they should be provided (by their supervisor) with a Workers Compensation Package (available from the Human Resources Manager). If the injury results in lost time for the employee, or is serious in nature, the Human Resources Manager should be notified immediately (as they will be obliged to notify WorkCover). Ensure that: A risk assessment is conducted and reported using Simulab Risk Assessment Form 05-108. Identified risks are placed in the risk register, any identified risks are communicated to others in the workplace. 02-002
