Self-Study Report Submission
Undergraduate Program Review
Copies of the undergraduate program review self-study report should be sent to
the Executive Vice President/Provost and the Assistant Vice-Provost of
Institutional Effectiveness, Planning and Research at least four weeks prior to the
reviewer’s campus visit. This signature sheet should be attached to each copy of
the report.

has been reviewed and approved by the Department Chair and the Dean of
the College.
