Thank you for your application for renewal in the Environmental Laboratory Accreditation
Program. Enclosed is a Certificate of Accreditation covering the one-year period beginning
October 12, 2015 and a current Scope of Accreditation.
Renewal of accreditation is based in part on review of your lab’s performance over the past year
as evidenced by participation in proficiency testing (PT) studies.
Accreditation is based in part on recognition of third party accreditation with Oregon
Environmental Laboratory Accreditation Program.
Accreditation is based in part on recognition of third party accreditation with DoD-ELAP.
Non-potable water accreditation was denied for Chloroprene, 2-Picoline, Epichlorohydrin, and pDioxane by EPA 624 because they were not listed on the Labs third party scope of accreditation.
Non-potable water accreditation was denied for Biphenyl by EPA 625 because it was not listed
on the Labs third party scope of accreditation.
Solid and Chemical Materials accreditation was denied for m+p xylene and for o-xylene by EPA
8021B because they were not listed on the Labs third party scope of accreditation.
Please remember when submitting any supporting documents, such as revised QA Manuals,
method SOPs, etc., submit them electronically, rather than hard copy, whenever possible.
Please also keep in mind the 3 key elements to include for every PT study. These items are
critical to the error-free uploading of your study results into our database:
1) Your “Participant Identifier” aka “EPA Lab ID”.
2) Your Method Codes. These have been updated in your new Scope of Accreditation to
the current EPA approved methods per the Federal Register published May 18, 2012. You
will find these listed on your next Add/Delete form included with your renewal
application packet. These method codes are specific to your fields of testing and scope of
accreditation. The codes are available on our website by lab.
3) The Method Descriptions. These have been updated in your new Scope of Accreditation
to the current EPA approved methods per the Federal Register published May 18, 2012.
These are also listed on your next Add/Delete form and are available on our website.
Please be sure your procedures are in compliance with the current versions of the methods
(including revising your SOPs and/or QA Manual).
As a reminder, continued participation in the Ecology Lab Accreditation Program requires the lab
to:

Submit a renewal application and fees annually.
Report significant changes in facility, personnel, analytical methods, equipment, the lab’s
quality assurance (QA) manual or QA procedures as they occur.
Participate in proficiency testing studies semi-annually, with the following exception: For
each parameter where all PT results were satisfactory, you are required to submit only one
PT result over this next year, and in subsequent years, as long as the results are satisfactory.
(This new allowance went into effect on September 9, 2010.)
Submit copies of current third-party Scopes of Accreditation when they are available.
YOUR RIGHT TO APPEAL
You have a right to appeal Ecology’s decision to the Pollution Control Hearing Board (PCHB) within 30 days of the
date of receipt of this decision letter. The appeal process is governed by Chapter 43.21B RCW and Chapter 371-08
WAC. “Date of receipt” is defined in RCW 43.21B.001(2).
To appeal you must do the following within 30 days of the date of receipt of this decision:

File your appeal and a copy of this decision with the PCHB (see addresses below). Filing means actual
receipt by the PCHB during regular business hours.
Serve a copy of your appeal and this decision on Ecology in paper form - by mail or in person. (See
addresses below.) E-mail is not accepted.
