Thank you for your application for renewal in the Environmental Laboratory Accreditation
Program. Enclosed is a Certificate of Accreditation covering the one-year period beginning May
13, 2014 and a current Scope of Accreditation.
Renewal of accreditation is based in part on review of your lab’s performance over the past year
as evidenced by participation in proficiency testing (PT) studies.
Accreditation is based in part on Third Party Accreditation with Oregon State Public Health
ORELAP Accreditation.
Method EPA TO-17 SIM was denied Third Party Accreditation since it was not listed on the
Oregon Scope of Accreditation.
Please remember the following:
If your lab analyzed the required amount of PT samples for any given parameter for this past
accreditation year with all results acceptable, you only need to submit one acceptable PT result
for that parameter for the next accreditation period.
When submitting any supporting documents, such as revised QA Manuals, method SOPs, etc.,
submit them electronically, rather than hard copy, whenever possible.
As a reminder, continued participation in the Ecology Lab Accreditation Program requires the
lab to:

Submit a renewal application and fees annually.
Report significant changes in facility, personnel, analytical methods, equipment, the lab’s
quality assurance (QA) manual or QA procedures as they occur.
Participate in proficiency testing studies semi-annually, with the following exception: For
each parameter where all PT results were satisfactory, you are required to submit only
one PT result over this next year, and in subsequent years, as long as the results are
satisfactory. (This new allowance went into effect on September 9, 2010.)
Submit copies of current third-party Scopes of Accreditation when they are available.
YOUR RIGHT TO APPEAL
You have a right to appeal Ecology’s decision to the Pollution Control Hearing Board (PCHB) within 30 days of the
date of receipt of this decision letter. The appeal process is governed by Chapter 43.21B RCW and Chapter 371-08
WAC. “Date of receipt” is defined in RCW 43.21B.001(2).
To appeal you must do the following within 30 days of the date of receipt of this decision:

File your appeal and a copy of this decision with the PCHB (see addresses below). Filing means actual
receipt by the PCHB during regular business hours.
Serve a copy of your appeal and this decision on Ecology in paper form - by mail or in person. (See
addresses below.) E-mail is not accepted.
You must also comply with other applicable requirements in Chapter 43.21B RCW and Chapter 371-08 WAC.
