PG B 481 (5-7-80) Policy and Procedures for the Issuance of Work Permits 1.Policy Work permits may be issued to students who are under age 18, who have not graduated from high school, and who have been admitted to or enrolled in one of the colleges of the Los Angeles Community College District. A work permit is not required for high school graduates. The responsibility for compliance with state and federal laws lies with the president or designated representative of the college. 2.Procedure for the Issuance of Work Permits a. The college office responsible for the issuance of work permits will issue Calif. State Dept. of Education form B11, "Request for Work Permit and Statement of Intent to Employ Minor." The front side of B1-1 form is to be completed and returned to the college. b. The college will issue Calif. State Dept. of Educ. Form B1-4,”Permit to Employ and Work.” c. The work permit should be stapled to Form C1074, "Certification of Information, Unclassified Service." d.
