Laboratoryexerciseswillcomprise70%ofyourgradeinthiscourse.Eachlabmaybedevelopedbyindividual
facultymember(s)andmayconsistsofanundeterminednumberofpoints.Eachlabwillbeweightedequally
forthesemester(withtheexceptionoflabsrelatedtoimmunizations).
Labquizzeswillcomprise10%ofyourgradeinthiscourse.Quizzeswillbegivenatthebeginningofclassand
willbetimedbytheinstructor.Tardystudentswillnotbegivenextratimetocompletethequizzes.Allwork
mustbeendedwhentheinstructorcallsfortimeofsubmission.Studentsnotsubmittingquizzesimmediately
afterbeingrequestedtodosowillbegivenagradeofzero(0)forthatquiz(i.e.quizzesturnedinlatewill
resultinagradeofzero).Allquizzeswillbetakeninclass,unlessotherwiseinstructedbytheprofessor.
Quizzestakenoutsideofclasstimewillresultinagradeofzero.
Quizcontentwillreinforcetheutilizationofapackageinsertandotherdrugreferences,calculationsneeded
fornormaldosing;renaladjustment;andI.V.dripratesforadult,pediatric,andneonatalpatients,andmay
alsoincludeinformationrelatedtoimmunizations.
ILS-VIwillalsocontainnewmaterialintheareaofimmunizations,whichwillcomprise10%ofyourgradein
thiscourse.TheAmericanPharmacist’sAssociation“Pharmacy-BasedImmunizationDelivery”certification
programwillbeused.Thisisaninnovativeandinteractivetrainingprogramthatteachespharmacists(and
students)theskillsnecessarytobecomeaprimarysourceforvaccineinformationandadministration.The
programteachesthebasicsofimmunologyandfocusesonpracticeimplementationandlegal/regulatory
issues.
Therearethreecomponentstothecertificate-trainingprogram:
• Self-studymoduleswithcasestudiesandassessmentexam
• Liveseminarwithfinalexam
• Hands-onassessmentofintramuscularandsubcutaneousinjectiontechnique
ACertificateofAchievementisawardedtoparticipantswhosuccessfullycompleteallactivityrequirements,
whichincludetheself-studycomponent,livetrainingseminar,andtheinjectiontechniqueassessment.
Successfulcompletionisdefinedasascoreof70%orbetteronboththeself-studyandliveseminar
assessments.
1. OnlineSelf-StudyModules:Ascoreof70%orbetterisrequired(innomorethantwoattempts)PRIOR
toattendingthelivetrainingseminar.Iftheself-studyexamisnotcompletedbythefirstdayofthe
liveseminar,20pointsperdaywillbedeductedfromyourlabgradeuntiltheself-studyiscompleted
orascoreofzeroisobtained.
Theonlineself-studymodulesandassessmentarerequiredforsuccessfulcompletionofILS-VI,andalso
requiredforAPhAPharmacy-BasedIMZtrainingcertificate.
2. LiveSeminar:Willbeconductedovertwolabperiods.Attendanceisrequiredforallstudents.
3. OnlineFinalExam:Ascoreof70%orbetterisrequiredbytheduedatesetforthbytheinstructor.
Participantsmayhaveamaximumoftwoattemptstoachieveapassingscore.Thiscomponentisa
requirementforsuccessfulcompletionofILS-VI,andalsorequiredforAPhAPharmacy-BasedIMZ
trainingcertificate.
4. Assessment:Demonstratecompetencyinonesubcutaneousandtwointramuscularinjectionsas
assessedbytheactivityfacultyduringthelivetrainingseminar.ThiscomponentisrequiredbyAPhAin
ordertoearnaCertificateofAchievement.PointswillnotbeattachedtothiscomponentforILS-VI.
5. NOTE:TheCertificateofAchievementisinvalidwithoutwrittenproofofcurrentCPRorBCLS
certification.Inaddition,allrequirementsbytheLouisianaStateBoardofPharmacymustbemetprior
toengaginginimmunizationpractice.
***YouwillberequiredtouploadacopyofyourMedicationAdministrationRegistrationfromtheLouisiana
BoardofPharmacy(LABP)toExamSoftasanartifactforlab.Inordertodothis,youmustapplytotheLABP
bycompletingForm19ontheLABPwebsiteandsubmittingthisalongwithacopyofyourAPhAImmunization
CertificateofAchievement(COA),andacopyofyourcurrentBLScard.Whenyourcredentialshavebeen

StudentPortfolios:
Studentsarerequiredtoprepare,maintain,andsubmitaportfolioasevidenceofachievementoflearning
outcomes.Thisportfolioalsoreflectsthestudent’sevolvingprofessionalgrowthfromthebeginningtothe
completionoftheDoctorofPharmacyprogram.
Goalsoftheportfolioprocess:
1.Documentstudents’progressiveachievementoftheschool’scompetenciesthroughoutthe
curriculumandpracticeexperiences.
2.Documentstudents’self-assessmentandpreceptorassessmentsofeducationaloutcomes.
Theportfoliomustcontaintherequireditems(artifacts)bytheendoftheSpringsemesteroftheP3year.
Informationregardingtherequiredartifactsforthissemesterwillbegiventoyouatthebeginningofthe
semester.
Completionofasatisfactoryportfolioisarequirementtoadvancetothenextsemester.Attheendofeach
semester,reportswillbegeneratedbytheDeanofAssessmenttoensurethateachstudenthassubmittedthe
appropriateartifacts.Studentswhohavenotsubmittedtherequiredartifactswillbenotifiedofdeficiencies,
andwillreceiveanincompletegrade(“I”)forthiscourseunlessthedeficienciesarecorrectedbyaset
deadlineoftwoweeksfollowingthecompletionofthesemester.PertheULMStudentPolicyManual“I”
gradesareremovedonlybycompletionofthecoursework,notbyrepeatingthecourse.Thedeficiencymust

B. AttendancePolicy:Classattendanceismandatoryinallpharmacycourses.Studentsreportedfor
accumulatingmorethanthreeunexcusedabsencesinacourseduringanacademicsemesterwillbe
administrativelydroppedfromthecoursewitha“W”grade.InaccordancewithSchoolofPharmacy
policyandprocedure,agradeof“W”willbecountedasan“F”gradewithrespecttoacademic

STUDENTSMISSINGAGRADEDEXERCISE
Astudentmissingagradedexercise(exam,quiz,in-classassignment,scheduledlabetc.)MUSTcontact
theCourseCoordinatorviaemailorphonepriortothegradedexercise.Ifastudentcannotcontact
theCourseCoordinatorpriortothegradedexercise,theymustcontactthecoordinatorwithin24
hoursofthegradedexercise.ItwillbethesoleresponsibilityoftheCourseCoordinatorasto
determinewhetherornotthestudenthadasufficientexcusefornotcontactingtheCourse
