(0)forthatquiz.StudentsmissingtheTuesdaydeadlinewillbegivenagradeofzero(0)unlessyouhave
aUniversityapprovedexcuse.MaterialforTop200drugquizquestionswillcomefromthecurrent
editionofSigler’sPrescriptionDrugCards.Eachstudentwillberequiredtopurchaseasetofthesecards
whichwillbeutilizedthroughoutthecurriculum.(Note:Thepurchaseofupdateswillbenecessaryin
subsequentyears).ThesecardscanbepurchasedthroughtheULMBookstore,ordirectlythroughSFI
MedicalPublishing.Informationcontainedonthesequizzeswillconsistofthefollowing:brandname,
genericname,dosageform(s),indication,andtherapeuticclass.Youwillbeallowedtodropyour
lowestTopDrugquizforatotalof10quizzestobecountedinyourfinalgrade.Ifyoumissaquiz,this
willbecountedasoneofyourdropquizzesunlessyouhaveavalidatedUniversityapprovedexcuse.If
so,youcanmakeupthequizonthelastweekofschool(April2–May6)atatimeconvenientfor
professorandstudent.
Mid-Term&FinalExam
TherewillbeaMedicalTerminologymid-termexamandacomprehensivefinalexam.Bothexamswill
begivenusingExamSoft.Materialwillcomefromyourchapterreadings,anddrugcards.
Scheduledquizzescanbefoundontheteachingschedulebelow.Thefinalexamwillbetheweekprior
tofinalsonMay3rdat10:00am.
Eachquizorexammaycontainamixtureofquestiontypesandmayincludemultiplechoice,true/false,
fillintheblank,matching,andshortanswer.NOinformationalresources(i.e.classnotes,textbook,
internet,electronicstoragedevice,etc.)ofanykindaretobeusedduringanexaminationunless
otherwiseinstructed.Anystudentearninganon-passinggradeof“D”or“F”onthemid-termexamwill
berequiredtoparticipateinmandatorytutoringsessionofferedbythecourseinstructorpriortothe
finalexam.

(Mostrecenteditionwithupdates).Laptopisrequired.
B. AttendancePolicy:Classattendanceismandatoryinallpharmacycourses.Studentsreported
foraccumulatingmorethanthreeunexcusedabsencesinacourseduringanacademicsemester
willbeadministrativelydroppedfromthecoursewitha“W”grade.InaccordancewithCollege
ofPharmacypolicyandprocedure,agradeof“W”willbecountedasan“F”gradewithrespect
toacademicstandards.Tardinessanddisruptivebehaviorwillnotbetolerated.
EXCUSEDABSENCESPOLICY
STUDENTSMISSINGAGRADEDEXERCISE
Astudentmissingagradedexercise(exam,quiz,etc.)MUSTcontacttheCourseCoordinatorvia
emailpriortothegradedexercise.IfastudentcannotcontacttheCourseCoordinatorpriorto
thegradedexercise,theymustcontactthecoordinatorwithin24hoursofthegradedexercise.
ItwillbethesoleresponsibilityoftheCourseCoordinatorastodeterminewhetherornotthe
studenthadasufficientexcusefornotcontactingtheCourseCoordinatorpriortotheexam.
Providedthispolicyisfollowedandavalidatedexcuseispresented,excusedabsenceswillbe
grantedforthosereasonsoutlinedintheUniversitycatalogandSchoolofPharmacy(SOP)
StudentHandbook.AbsencesoutsideofthosecoveredintheUniversitycatalogandSOP
StudentHandbookwillbeexcusedatthediscretionoftheCourseCoordinator.
STUDENTSMISSINGCLASSTIME
StudentsMUSTnotifyfacultyofascheduledabsence(PhysicianAppointment,etc)priorto
missingclass.Whenpossible,studentsSHOULDnotifyfacultyofanunscheduledabsenceby
phoneoremailpriortomissingclass.IfastudentcannotcontacttheCourseCoordinatorprior
toclass,theyMUSTcontactthecoordinatorwithin24hoursofclass.Providedthispolicyis
followedandavalidatedexcuseispresented,excusedabsenceswillbegrantedforthose
reasonsoutlinedintheUniversitycatalogandSOPStudentHandbook.Absencesoutsideof

StudentsSHOULDNOTmissclassfordrugscreening;however,intherarecasewherea

window,anexcusedabsencewillbegrantedbytheDeanorhis/herdesigneeprovidedthe
studentreceivespermissionfromtheDean’sofficeortheCourseCoordinatortobeabsentprior
totheabsenceoccurring.ThisexcusewillbecommunicatedinwritingbytheDeanorhis/her
designeedirectlytotheCourseCoordinator.
EXCUSEVALIDATION
ThevalidityofallexcuseswillbeverifiedbytheOfficeofStudentandProfessionalAffairs.
StudentsshouldbringtheexcusetotheOfficeofStudentandProfessionalaffairsbeforeclasses
onthedaytheyreturntoclass.TheOfficeofStudentandProfessionalAffairswillverifythe
validityoftheexcuseandwillsignanddatetheexcuse.Thestudentshouldretrievethe
validatedexcuseontheSAMEdayitisdroppedoffintheofficeofStudentandProfessional
Affairs.ThestudentshouldprovidetheCourseCoordinatorwiththevalidatedexcusewithintwo
businessdaysofitsvalidation.
