This session will begin by providing the audience with an overview of the significance of medication errors (or medication incidents) in the health care system. The facilitator will provide an overview of medication safety and its relationship to continuous quality assurance in community pharmacy practice and go through the different response to medication errors – the person approach versus the systems approach.
Participants will also learn about the ISMP
Canada Medication Safety Self-Assessment
(MSSA), Community Pharmacy Incident
Reporting (CPhIR) Program, medication incident reporting and analysis, as well as error prevention principles.

Speaker Biography
Certina Ho, R.Ph., B.Sc.Phm., M.I.St.,
M.Ed.
A graduate of the Faculty of Pharmacy, University of
Toronto, Certina Ho is currently a Project Manager at the Institute for Safe Medication Practices Canada
(ISMP Canada) and Adjunct Assistant Professor at the School of Pharmacy, University of Waterloo.
In addition, she has experience in community pharmacy practice and management of internet health content. Certina obtained her graduate degrees in Library and Information Science in 1998 and in Education in 2005. In her current position,
Certina is responsible for medication safety initiatives related to community pharmacy practice as well as medication incident reporting and analysis at ISMP
Canada.
