JOB DESCRIPTION
TITLE: Social Work Assistant
POSITION INFORMATION: Assists in the implementation of social programs by conducting social work, research,
developing histories, conducting follow-up, consulting with Social Workers and other healthcare personnel, and maintaining
records.
REPRESENTATIVE DUTIES:
1.
Social Work. Provides direct service to consumers and educational support to families of consumers and staff. Coleads consumer and caregiver support groups.
2.
Research. Participates in on-going research activities.
3.
History Development. Develops social histories on selected cases from interviews, consumers, or significant others.

Follow-Up. Initiates follow-up with consumers, caregivers, and significant others. Assists consumers with
arrangements of respite care services.
5.
Consultation. Consults with Social Worker and other healthcare personnel in the development of program
assessment and procedure evaluations. Participates as a team member in multidisciplinary teams by attending
meetings to ensure the best care is being provided.
6.
Record Maintenance. Maintains appropriate clinical records.
7.
As Needed. Performs various duties as needed to successfully fulfill the function of the position.
REQUIREMENTS:
Education: Bachelor’s Degree in Social Work or Social Science
Experience:
Certifications/ Licenses:
Equivalent/ Substitution: Will accept 48 months of equivalent experience in lieu of a Bachelor’s degree.
