PARENTAL AGREEMENT
I, ___________________________, the parents/guardians of ___________________________ understand and
agree to the following (please initial to indicate agreement):
_____ I will pay full tuition fee on the first day of each week/month/semester as described in the Parent
Handbook. A Late Fee of $5.00 is charged each week payment is late.
_____ I will pay a Late Pick-Up Fee if my child is picked up late from the Center. The fee is $15.00 for every 15
minutes or portion thereof late (note policy details in Parent Handbook).
_____ I will read and follow the procedures in the Parent Handbook (including, health and illness policies,
vacation policies, late pick-up policies and tuition policies.)
_____ I agree that, if first aid is needed for my child, it will be administered by Early Childhood Staff or C.O.D.
Police Department Staff. In case of extreme emergency, I understand that C.O.D. Police staff will contact the
local ambulance service, which will transport my child to the nearest hospital for treatment.
_____ The Early Childhood Demonstration Center is an observation/participation site for the Early Childhood
Education college students as well as students from other departments within COD. College students will
periodically be observing all enrolled children but will never be alone with children.
_____ Walking field trips on campus are an integral part of the curriculum and will take place without advance
notice or separate permission. A field trip away from the campus will be announced in advance and a separate
written parental permission will be required.
_____ The teachers routinely use photographs and videos of the children as part of the curriculum. In addition,
the COD Public Relations Department may periodically take pictures or videos of enrolled children for publicity
purposes or for the college website. Children may be identified. IF YOU OBJECT TO YOUR CHILD'S
PICTURE OR VIDEO BEING USED FOR PUBLICITY OR TO YOUR CHILD BEING IDENTIFIED, YOU MUST
NOTIFY THE CENTER, IN WRITING, WITHIN ONE WEEK AFTER ENROLLMENT.
_____ Each family's name, address, telephone number and e-mail address are included on a class list to be
distributed ONLY to Early Childhood Demonstration Center teachers and families. IF YOU WANT YOUR
INFORMATION DELETED FROM THE LIST, YOU MUST NOTIFY THE CENTER, IN WRITING, WITHIN ONE
WEEK AFTER ENROLLMENT.
_____ ALL FAMILY RECORDS ARE KEPT CONFIDENTIAL. Only teachers and Center administrators have
access to files. Files may also be reviewed by our Illinois Department of Children and Family Services Licensing
Representative during a licensing visit or our DuPage County Health Department Nurse during a medical records
check. No information will be released to any other person or agency without parent/guardian's written
permission.
_____ Center staff must report all cases of suspected child abuse or neglect to the Illinois Department of
Children and Family Services as required by state law.
_____ The College of DuPage Early Childhood Demonstration Center reserves the right to disenroll any child if
parental responsibilities to the center are not met. Any child who, after attempts have been made to meet the
child's individual needs, demonstrates the inability to benefit from the type of care offered by the Center, or
whose presence is detrimental to the group, may be discharged from the Center. This will be done after
confidential conferences between parents, teachers and administrators.
