Guide 13 Non-Catalog Products Under 5k The Non-Catalog Products Under $5000 form is designed to streamline the purchase of products for purchases less than $5000. This form will create 1 line item within your requisition. For multiple line items, use additional non-catalog <$5K forms as applicable. Click on the link to open the form. Note that the University’s preferred method of payment is via ProCard which is done outside the system. In the event you will not be using your Procard for this order please follow the guide below. Note that the Vendor field is an active search such that the system begins searching the vendor records as you type in the name. Once you see the name of the vendor you are seeking display, click it If the vendor you are seeking does not display after typing it in, click enter manually. Enter the vendor name, phone, fax, email. You will also need to select a distribution method. Note that if you intend to use your Procard as payment on the requisition, you must only select the fax check box and type in the vendor fax number; otherwise, select the email checkbox and type in the vendor email address. Please also make sure that you attach a completed substitute W9 form at the bottom of the screen. After you have found your vendor, enter the Product Description, Qty and (OPTIONAL) type of packaging. Note the field preceding the UOM packaging is used to denote how many in the packaging type you select. For example, you need 1 case and each case holds 12. Put 1 for quantity, select CS as your UOM and enter 12. Enter the manufacturer name and part number. If the item you are ordering is a Health and Safety item, please check the applicable check box. If you have a written quote from the vendor (this is preferred) – Attach to the bottom of this form Once you have entered the fields for this form, ensure the dropdown at the top right of your screen reads “add and go to Cart”. Select the Go button. Should you require multiple line items on your requisition. Add a Non-Cat under $5K form for each line item.
