Fitness Testing – AFIT students attending the Naval Postgraduate School (NPS)
It is every Airman’s responsibility to maintain the AF Fitness Program standards 365 days a year (AFI 36-2905)
Know when you are due because it is your responsibility to schedule your test.
Your AFIT Training Report (TR) annotates Fitness Status. If you fail or if you are overdue the TR will be a referral, and
will annotate “Does not meet standards” or “Not current”.

Profiles
a. If you have a profile, you must bring/send a copy to Capt Franklin and SrA Davis at
david.franklin@goodfellow.af.mil and phylicia.davis@goodfellow.af.mil (CML 242-5026)
b. They will translate the profile onto an AF Form 422.
c. Send a copy of the 422 along with your questionnaire to the Commander, AFIT Force Support,
NPS. You cannot schedule a test without the 422.
e. Even if you have a profile, you must still take the test (all non-exempt components) in your due month or
you will be overdue. REMEMBER if you are overdue at the time of your Training Report closeout, your
TR will annotate your “Not Current” status and it will be a referral TR.

*Electronic Fitness Screening Questionnaire (eFSQ – complete NLT 7 days, and NET 30 days, prior to test)
a. Form completion requires access to a computer with CAC reader and Adobe Acrobat version 7 or higher.
Convenient locations include the NPS library or most campus computer labs.
b. Scroll to Page 2 of this document. Complete questions 1 – 4, and follow the written instructions.
c. At the bottom of Page 2, complete all entry fields (Name, Rank, SSN, etc). These are mandatory fields.
d. Insert CAC into reader and click the Member Signature field. From the list, select your U.S. Government
Digital Signature Key. (see bottom of this page for troubleshooting). Adobe will ask you to save the document
– consider saving to a location where you can conveniently access the form, such as your desktop. Once you save
the document and enter your PIN the document will be digitally signed.
e. E-mail the signed form to wmrudd@nps.edu and attach a copy of your fitness profile to the e-mail, if
applicable. Once you have sent the e-mail you should consider deleting the saved form from the hard drive to
secure your personal information.

Testing slot
a. When you e-mail your eFSQ to Maj Rudd, give him your preferred testing date/time. Reservations must be
confirmed Friday the week prior to your requested date.
b. Sessions are available on most Fridays at 0800
c. When you confirm your testing time and receive the signed eFSQ, you are ready to test.

Testing Day. Report to the POM Fitness Center. Bring the following items:
Air Force PT Uniform is required
Copy of Completed Fitness Questionnaire and Score Sheet
Your Military ID Card
AF Form 422 if applicable (Profile Document)
*Troubleshooting:
CAC Certificate is not available in Adobe: Make sure that your CAC reader and ActivClient are installed. Open ActiveClient
by right-clicking its icon in the system tray. Right click My Certificates and select Make Certificates Available in Windows.
“The Windows Cryptographic Service Provider reported an error : the security to ken …” (or similar error): Ensure
that you are selecting the Digital Signature Key from your CAC card and not another certificate, such as your ID or e-mail
certificate.

Answering these questions honestly is in your best interest.
1. Have you experienced any of the symptoms/problems listed below and not been medically evaluated and cleared for unrestricted participation in a
physical training program?
- Unexplained chest discomfort with or without exertion
- Unusual or unexplained shortness of breath
- Dizziness, fainting, or blackouts associated with exertion
- Other medical problems that may prevent you from safely participating in this test
Yes: Stop. Notify your UFPM and contact your PCP/MLO for evaluation/recommendations (or for ARC, contact the MLO for Duty
Limiting Conditions (DLC) documentation and referral to PCP).
No: Proceed to next question.
2. Are you 35 years of age or older?
Yes: Proceed to next question.
No: Stop. Sign form and return to your UFPM. Member may take the fitness assessment.
3. Have you engaged in vigorous physical activity (i.e., activity causing sweating and moderate to marked increases in breathing and heart rate)
averaging at least 30 minutes per session, 3 days per week, over the last 2 months?
Yes: Stop. Sign form and return to your UFPM. Member may take the fitness assessment.
No: Proceed to the next question.
4. Do one (1) or more of the following risk factors apply to you?
- Smoked tobacco products in the last 30 days
- Diabetes
- High blood pressure that is not controlled
- High cholesterol that is not controlled
- Family history of heart disease (developed in father/brother before age 55 or mother/sister before age 65)
- Age > 45 years for males; > 55 years for females
Yes: Stop. Notify UFPM.
- RegAF: If member was cleared for entry into a fitness program at their last PHA and their PHA is current, the member will take the
fitness assessment. If not cleared, member will be referred to PCM for evaluation, and, if medically cleared for unrestricted fitness
program, the member will take the fitness assessment.
- AFR: If member was cleared for entry into a fitness program at a PHA within the last 12 months, the member will take the fitness
assessment. If not previously cleared, member will be referred to PCP for evaluation, and, if medically cleared for unrestricted
fitness program, the member will take the fitness assessment. Refer to MLO if there is any combination of smoking, diabetes,
uncontrolled high blood pressure, and/or uncontrolled high cholesterol. MLO will update medical records and/or initiate DLC
documentation. Member begins a one-time 90-day Fitness Assessment Deferral and is provided Fitness Assessment Deferral
Guidance (Attachment 5).
- ANG: Member begins a one-time 90-day Fitness Assessment Deferral and is provided Fitness Assessment Deferral Guidance
(Attachment 5). Refer to MLO if there is any combination of smoking, diabetes, uncontrolled high blood pressure, and/or
uncontrolled high cholesterol. MLO will update medical records and/or initiate DLC documentation.
No: Stop. Sign form and return to your UFPM. Member may take the fitness assessment.
If member experiences any of the symptoms listed in Question #1 during the fitness assessment, they should stop the test immediately and
seek medical attention immediately.
