APPLICATION
•Must be a U.S. resident enrolled in a university
or community college that is accredited by the
U.S. Department of Education and is located in
the United States at the time of award.
•Must be a cancer patient, cancer survivor
and/or amputee.

*The Community Foundation of Central
Wisconsin awards over 120 scholarships to
students every year for educational financial
support. Most of the scholarship funds have
been established as endowments, so students
will continue to benefit long after the donor's
lifetime
*Each scholarship may have its own eligibility
requirements and criteria
*Many scholarships are limited to residents of
specific counties or graduates of specific high
schools.
*The Eric Delson Memorial Scholarship is
intended for college, vocational or private
school students that are clinically diagnosed
with hemophilia or von Willebrand disease.
*US Citizen and students with an Alien
Registration number with current enrollment in a
state-approved schools of nursing or pre-nursing
in associate degree, baccalaureate, diploma,
generic doctorate, and generic master's programs
*RN to BSN/MSN and LPN/LVN to RN
students are also eligible
*Scholarships are based on academic
achievement, financial need, and involvement in
nursing student organizations and community
activities related to health care
*Additional criteria may be required by some
sponsors
*U.S. resident
*Between 17-24 years of age
*Experienced the death of a parent or legal
guardian to cancer
*Enrolled in college
*Be attending college or university no later than
December of 2016.
*Be attending college or university on a fulltime basis by December of 2016.
*Be currently in your last year of high school or
a freshman, sophomore, or junior in
college/university.

*Applicant must be a current/former registered
VOLUNTEEN/VOLUNTEER or current
EMPLOYEE of Ministry St. Joseph’s Hospital
in Marshfield, Wisconsin.
*Applicants must be enrolled or enrolling in a
program of higher education.
*Scholarship funds must be spent on tuition.
*Financial need is considered but not
mandatory.
* A student is eligible to receive a Partners
Scholarship twice.
•Diagnosed at or before age 19 with a primary
malignant or nonmalignant central nervous
system brain and/or spinal cord tumor, as
identified by the World Health Organization.
•A completed online application with all
necessary documents (signed physician’s letter
confirming diagnosis, signed medical release
form and school transcript) and one
recommendation letter, by April 20, 2015 at 5
p.m. Eastern.
•Plan to attend or already attending an
accredited academic institution within the
United States
Several Scholarship opportunities are available
with varying awards, eligibility requirements
and deadlines.
*Applicant must be a resident of Portage County
*Proof of enrollment or acceptance for the fall
semester at a university, college or technical
school.
*Must be pursuing a post-secondary degree in:
Nursing, Dietetics, Health Education, or
Environmental Health
*Applicants must be enrolled or intending to
enroll for a minimum of 6 credits per semester.

*Must be pursuing a career in nursing or an
allied medical field.
* Must be enrolled full-time in the 2nd, 3rd, or
4th year of training or entering a training
program after 1 year of prep work in the chosen
field.
* Must be a resident of the State of WI (could be
attending a school in another state).
*Is eligible to receive this scholarship only once.
* A picture of the applicant must accompany this
application. (The picture is to be used for public
relations purposes)
*A separate letter must accompany this
application. It should explain why you have
chosen Nursing or an Allied Medical Field as a
career. Please keep to a 200 word maximum.
