Addendum: The purpose of the addendum is to acknowledge changes to the following items: 1) The Grading scale on the Triad form has been changed to create consistency among the various forms that are used to evaluate practicum and student teaching interns. We are moving from a 1-4 scale to a 0-3 scale. This has now been aligned to match the form submitted on TaskStream for a seamless transition. 2) The second to last page of the form was asking for information that is no longer available due to the new observation forms and the way the students are assessed. The new addition to the Triad form allows for the Supervisor and the Mentor to collaborate and then average both scores onto one form. The mentor and the supervisor are each still supposed to fill out their respective forms, this just allows for better transparency when the forms are turned into the Office of Field Services.
