Adult Services - Adult Care Home Case Management/Enhanced Care
Primary Purpose and Duties of the Position:
Primary Purpose of position is directed at goal of improving overall quality of care for Special
Assistance/Medicaid eligible, heavy care residents of Adult Care Home and Homes for Aged. Service
provides support to residents who are seriously impaired and require extensive assistance in order to have
their needs adequately addressed. Extensive assistance is considered necessary when a resident needs
assistance with ambulation, toileting and or eating three or more times a week. This individual may work
with up to thirty-five heavy care residents which require assistance with ambulation, toileting and or
eating.
After assessment has been completed, case manager needs to begin setting goals and develop service plan
with client. Case manager, along with significant others, family members, adult care home staff and other
responsible parties should be involved in developing service plan. Case manager must contact adult care
home staff and resident, family members, or another responsible party at least monthly to assure services
are being received which are appropriate and adequate to meet resident’s needs and consistent with quality
care. Case manager must also review home’s care plan at lease quarterly. Case manager, together with
resident and adult care home staff, must determine whether resident’s needs are being met. Case manager
must verify that resident continues to meet Medicaid criteria for heavy care resident and authorize
continuation of Adult Care Home Enhanced Care at lease every twelve months.
Position provides case management for Enhanced Personal Care caseload. An Adult Care Home Case
Management service is directed at improving the overall quality of care. Service provides support to
residents who are seriously impaired and require more extensive assistance in order to have needs
adequately addressed.
Purpose of Adult Care Home Case Management Services is for case manager to work in partnership with
resident, resident’s family, significant others, adult care home and community service providers to assure
that needs and preferences of heavy care residents are met. Case managers have important and diverse
roles with residents including verifying resident’s eligibility for Enhanced Care Home Personal Care,
reviewing adult care home care plans to be sure these plans correspond to needs of residents, and reviewing
adult care home’s provision of services to assure changes in resident’s conditions are being addressed.
Case manager also conducts broader assessment which can identify need for other health and social
services which might benefit resident. Case manager develops plan and monitors plan monthly and
quarterly.
This position will provide back-up to Crisis Intervention Program and Food Pantry Program during heavy
usage periods. The position will also include Beeper Duty which may involve Adult Protective Services
and Guardianship case management. The Beeper Duty will occur on a rotation basis after regular business
hours including weekends.
Knowledge, Skills, and Abilities:
Considerable knowledge of social work principles, techniques, and practices and their application to
individual casework group work and community problems. Knowledge of the psychosocial, socioeconomic
and behavioral problems and their treatment. Knowledge of governmental and private organizations and
community resources. Knowledge of laws, regulations and policies which govern the program. General
knowledge of medical terminology, disease processes and their treatment is required in certain programs or
settings. Skill in establishing rapport with a client and applying techniques of assessing psychosocial.
behavioral, and psychological aspects of client's problem. Ability to establish and maintain effective
working relationships with administrative superiors, members of case load and their families, and with
civics medical, social and religious organizations. Ability to train or instruct lower-level social workers,
students, or interns. Ability to express ideas clearly and concisely and to plan and execute work.
Minimum Training and Experiment Requirements:
Bachelor's degree in social work from an accredited school of social work; Bachelor's degree in a human
services field from an accredited college or university and one year directly related experience; Bachelor's
degree from an accredited college or university and two years directly related experience.
*Directly related experience is defined as human services experience in the areas of case management,
assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment
planning. Degrees must be received from appropriately accredited institutions.
