R3 SUPPLEMENT 6509.11H-93-6 EFFECTIVE DATE: 10/03/1993 DURATION: This supplement is effective until superseded or removed 6509.11h_10 Page 2 of 2 FSH 6509.11H – SERVICE-WIDE CLAIMS MANAGEMENT HANDBOOK CHAPTER 10 – INCIDENT INVESTIGATIONS 11 - INCIDENT REPORTING REQUIREMENTS AND PROCEDURES. 11.21 - Fiscal Representation on Investigation Teams. The Regional Claims Officer will be a member of all accident review boards convened by the Regional Forester. 11.31 - Report Content. Form FS-6700-8, Report of Incident to Other Than Employees, and all pertinent documents, is to be provided to Fiscal and Public Safety (FPS). If the incident involves a vehicle accident, a copy of the form and attachments should also be provided to the Forest Engineer. 12 - INVESTIGATIONS. If an incident involves serious injury or death to either a private party or an employee, or significant property damage, The Regional Safety Officer will notify the Regional Claims Officer. As soon as the investigation has been completed, and it is determined that the possibility of a claim against the United States exists, send two copies of the investigative report to the Director of Fiscal and Accounting Services. 12.4 - Release of Investigation Reports. Do not release investigation reports for nonlaw enforcement situations to the public without specific consent of the Office of General Counsel. Nonlaw enforcement investigation reports include those prepared on civil matters such as fires, vehicle accidents and other property damage situations, or when an investigation is conducted on noncriminal matters. These reports are prepared to provide information to the Office of General Counsel and are, therefore, exempt from release under Exemption 5 of the Freedom of Information Act.
