CACFP Production Record Instructions Before the Meal 1. Site Information/Review of Requirements Indicate the site name, the name of the person completing the record and the date of the meal. 2. Meal Information List all menu items being served that contribute to the meal pattern (the bolded components are required). 3. Servings Prepared For each specific age group, document the number (#) of children, the appropriate serving size (i.e. cups, pieces, ounces, pounds, gallons, #10 cans, etc.) and the total amount needed. 4. Total Amount Prepared Document the total amount of each menu item prepared, by adding the amounts in all three grade groups together. After the Meal 5. Amount Leftover Document the total amount of food leftover for each menu item. 6. Actual Number Served In the top right-hand box, document the total amount of children served in each specific age group under the appropriate meal/snack.
