Tasks, Tips and Tools for Promoting Collaborative Community Teams - This resource identifies tasks for starting and sustaining collaborative teams along with tips for doing this effectively. It provides practical tools for carrying out these tasks and tips such as: sample forms and letters; formats for agendas, ground rules, and minutes; and step-by-step instructions for activities such as assessing the current community context, visioning, priority setting, action planning; and a generic interagency agreement format. Side-by-Side Description of Agency Responsibilities - This 3 column chart lists legal requirements under the Individuals with Disabilities Education Act (ID EA), Part C (fo r W V Bir th to T hree); IDEA, Part B (for schools), and Head Start Performance Standards. Requirements are organized according to 12 areas of collaboration: (1) family involvement; (2) child outreach/child find screening, referrals, and evaluations; (3) service eligibility; (4) individual program planning, e.g., Individualized Family Service Plans (IFSP) for children with disabilities ages Birth to 3 years and their families and Individualized Education Programs (IEP) for children with disabilities ages 3 through 5 years; (5) primary and related services; (6) service delivery in settings which, to the maximum extent possible, are in natural settings typical for the age of the child and which educate childr en w ith disabilities along with children without disabilities; (7) resource sharing which may include, but not be limited to, facilities, materials and equipment, collaborative services, screening, etc.; (8) transition; (9) confidentiality; (10) records transfer; (11) joint staff training; and (12) sharing child count data. W V Ea rly Childhood Comm unity C ollaboration Strate gies - This is a companion document to the legal side-by-side which identifies effective practice for community collaboration around the 12 areas of collaboration. Transition Checklist - This is a set of checklists that can be used by local agencies (WV Birth to Three, Ear ly Head Start, H ead St art, Public Sch ools, Child Care Programs and Other Comm unity Services) to track transition procedures and timelines in keeping with effective practice and/or legal requiremen ts. Use of this checklist is mandated by Policy 2525: West Virginia’s Universal Access to Pre-kindergarten System. IFSP/IEP Comparison - This provides a side-by-side comparison of (1) Individualized Family Service Plans required by WV B irth to Three and an option (with parent permission) for Pub lic School Special Educ ation for children with disabilities ages three through five years of age and (2) Individualized Education Programs required for children with disabilities who are provided special education and related services thro ugh the pub lic schools. Extended School Year Position Paper - This paper was developed by the WV Department of Education with WV Early Childh ood T ransition Steering Committee input. It describes options for addressing Extended S chool Year (ESY)/summer services issues for young children with disabilities who are provided special education and related by the public schools, including the services to children age 3 exiting WV Birth to Th ree services and entering public schoo l preschoo l special education services. Early Childhood Transition Brochure - This contains in formation on training, technical assistance and mater ials to help the WV B irth to Three System , Head Start, public sch ools and child care program s carry out their respective roles in transitioning young children ages birth to five years and their families from one program or service to another. Notice of Face-to-Face Transition Planning Meeting – This is a form letter that is required by the state WV Birth to Three system to be used by WV Birth to Three Service Coordinators in scheduling these me etings. Transition Planning Meeting Form – This is a form required by the state WV Birth to Th ree system to document the 90-Day Face-to-Face Meeting. Informa tion Sharing Form for Transition Planning – This form was developed by WV B irth to Three. It is required to be used with referrals from WV Birth to Three services as a cover sheet with referral information such as assessment reports, IFSPs, and other related information.
