Complete each question. If any item is not observed, the participant will receive an
incomplete for the entire in-service activity. One (1) lesson and one (1) student product is
needed for every ten (10) hours of in-service requested. A new form and observation is
required for every ten (10) hours requested (up to the maximum of 30 hours).
1. Describe the materials the teacher used from his/her travel to enrich the lesson.
2. How did the teacher connect the travel to the content, subject and/or SSS that he/she teaches?
3. What activities were students given to extend or reinforce the content presented?
4. Attach the lesson plan and a student product (1) from this lesson, demonstrating application or
knowledge of skills.
