The changes incorporated herein are an amendment to and supersede those conditions shown in the original solicitation. The Proposer shall acknowledge receipt of this addendum by signing this form and returning with their submittal. Failure to do so may subject Proposer to disqualification. DATE CHANGE: The tentative date for short list presentations has changed to June 29, 2015. Q1. Does the County have any requirements for contractors in regards to the Jessica Lunsford Act? If yes, please provide. A1. Under general conditions item 13, the requirements for Jessica Lunsford are listed. Please also reference Florida Statute 1012.465. Q2. Who currently holds this contract, and for how long? A2. Currently, vendors are awarded under RFQ 12-23. The vendors are Captec Engineering, Inc., Miller Legg, Sustainable Engineering 7 Design, LLC and Culpepper & Terpening, Inc. The initial term was awarded on August 28, 2012 for one year with 2 additional one year optional terms (which were exercised). Q3. What is the value of work & total number of work orders issued during the last contract period? A3. Since the work is “as-needed”, there is not an estimate of work. Contracts totaling $31,385 were awarded over the term of the current contract on four projects. Q4. Certificate of Insurance - In Section 3, item F of the RFQ states that “vendors shall furnish a certificate of insurance”. However, I do not see what tab in the response this should be included in OR Shall a certificate only be provided once the contract has been awarded? Please clarify. A4. Include the certificate in Section 4-I. Q5. Will preference be given to firms that have both Civil Engineering and Surveying? A5. The services requested are both civil engineering and surveying. Firms will be evaluated based on the criteria listed with the RFQ. Points are awarded based on the ability to provide requested services, see Section 4 of the RFQ.
