The Site Supervisor should complete this form and discuss the evaluation in a face-to-face meeting with the
fieldworker towards the end of the semester. The Evaluation should be given to the Faculty Supervisor before at the
completion of the fieldwork. This form is an important source of information for the assigning grades (Pass or No
Credit).
