of a minimum of 12 semester hours at UNI before recommendation to the Board of Educational Examiners will be made. With
this category you will be classified as a senior for registration purposes.)

I accept the fact that once enrolled as an undergraduate through this option, I may at any time seek admission to graduate study; but
once that is granted, I may not again revert to undergraduate student status until after the graduate program is completed. As a student
with a recognized baccalaureate degree, I understand that the credit earned while enrolled as an undergraduate will not be authorized
as graduate credit regardless of the level of the courses completed. I understand that all credit earned will be undergraduate credit and,
therefore, may never be used on a graduate degree program. I realize that failure to follow the program of study prepared for me may
cause the Registrar's Office to review and possibly alter my classification and registration status.
