An inherent risk of injury is assumed with the decision to participate in intercollegiate athletics/cheer unit (cheerleaders, precision dance, mascots). The University of Delaware seeks to provide the finest training, physical rehabilitation and medical care possible for its’ student-athletes and cheer unit. The training complex consists of the Bob Carpenter Center Training Room and the Delaware Field House Training Room. These facilities feature modern equipment and combine to form one of the finest training centers of its kind. Additionally, the University has a well-equipped Sports Medicine Clinic located in the Bob Carpenter Center as part of its’ overall Student Health Services. These services are guided by thoroughly trained professionals who are recognized experts in their field. Medical Certification for Individual Student-Athlete/Cheer Unit Participation The University of Delaware requires the studentathlete/cheer unit member to complete a physical examination at the Bob Carpenter Center Sports Medicine Clinic before that student is permitted to attend practice sessions or compete in any athletic event at the University. ANY TESTS OR CONSULTATIONS NEEDED TO GAIN APPROVAL FOR PARTICIPATION ARE THE RESPONSIBILITY OF THE STUDENT/PARENT/GUARDIAN. Upon completion of the physical examination the student will receive, from the Sports Medicine physician/nurse, a “certification for participation” form with two copies attached. These forms are to be distributed by the student to the student’s head coach and, where applicable, the equipment cage. No coach will permit an student to practice, nor will the equipment room issue any equipment, nor will the University provide insurance coverage to any student not following this procedure. This process must also be followed in order to use the Athletics Weight Room facilities, where applicable. If for any reason the student is not approved for athletic/cheer unit participation or practice, they will be so notified by the physician or nurse at the end of the physical examination. This procedure must be followed annually. If a student participates in more than one sport/cheer unit, or changes sports during an academic year, the student will need to complete a “recertification examination” at the beginning of the new season. To complete the medical recertification process, the student will need to make an appointment at the Sports Medicine Clinic for a review of his/her health chart and to receive a new certification form. Upon completion of eligibility in their final sport/season, students are requested to schedule an appointment with the Sports Medicine Clinic, prior to the conclusion of the academic term, for the purpose of an exit physical. This will include a review of the injuries received during participation and follow-up if needed. IF THE STUDENT ELECTS NOT TO RECEIVE THE EXIT PHYSICAL, THE UNIVERSITY WILL BE RELIEVED OF ANY LIABILITY AND/OR FINANCIAL RESPONSIBILITY FOR ANY DAMAGES OR INJURIES SUSTAINED AS A RESULT OF ATHLETIC/CHEER UNIT PARTICIPATION. General Student-Athlete/Cheer Unit Medical Care During the entire academic year, almost all medical needs of the student-athlete/cheer unit member will be treated at the Sports Medicine Clinic. The following support services are available at the Student Health Service in Laurel Hall: allergy injections, gynecological appointments, psychiatric services, alcoholism counseling and Health Education Services. The student should report to the Sports Medicine Clinic for general medical problems unless they become ill after the Clinic is closed, at which time they should report to the Student Health Service at Laurel Hall. The nurse on duty will see the student, but the student should follow up the next day at the Sports Medicine Clinic. In any case, the student who becomes ill should report in the morning to the Clinic so that the student’s coach can be notified early if the student will be unable to participate in practice. If the student discontinues participation, he/she is requested to inform the Sports Medicine Clinic. At that time the student’s health chart will be returned to the Student Health Service at Laurel Hall and the student’s future medical needs will be treated there. On occasion, the Sports Medicine Clinic allows students to borrow crutches. If they are not returned to the Sports Medicine Clinic within a reasonable amount of time, the student will be billed the cost of the crutches. Athletic Injury Medical Care Any certified intercollegiate athletic/cheer unit participant who receives an injury or becomes ill during approved participation periods must report the injury immediately to the Training Room or Sports Medicine Clinic to activate insurance coverage. Please note that the injury/illness must have been incurred in a practice or contest that was supervised by approved University coaching staff during a practice/contest period that was approved by the Director of Athletics and Recreation Services according to NCAA regulations, to be eligible for insurance coverage. The primary insurance carrier must be notified within 24 hours of injury by the student or his/her parent/guardian. If the student needs to see a physician outside the University staff, i.e. a specialist, that referral will be made by the Sports Medicine Physician. When consultation is requested or recommended, any recommendations for testing/surgery/repeat visits made by the consultant must be communicated to the University Sports Medicine Physician BEFORE any arrangements are made. If that referral results in a recommendation for non-emergency surgery, the student MUST inform the Sports Medicine Clinic PRIOR TO finalization of any arrangements. TO BE ELIGIBLE FOR UNIVERSITY MEDICAL COVERAGE, STUDENT-ATHLETES/CHEER UNIT MEMBERS MUST ARRANGE ALL SURGERY, TREATMENT, THERAPY AND CARE IN ACCORDANCE WITH REQUIREMENTS OF THEIR PRIMARY INSURANCE COVERAGE. For example, some out of state HMO’s and other Preferred Provider Groups and/or foreign insurance require a return to the home state or country for coverage to apply. Any costs not covered by insurance become the responsibility of the student-athlete/cheer unit member and/or his/her parent(s) or guardian(s). If the student decides to see a physician/specialist WITHOUT being referred by the Sports Medicine Clinic, the student will be responsible for all bills incurred. In addition, if the student elects a surgeon who is not approved by his/her medical insurance carrier, he/she is also financially responsible. Any student seen by an outside (off campus) physician, must return to the Sports Medicine Clinic for follow-up and final clearance prior to active participation status. In emergency situations, transportation is provided by the University’s Public Safety Department, otherwise it is the students responsibility to provide his/her own transportation. Training Room Policy All injuries and/or illness should be reported to the Training Room before visiting the Sports Medicine Clinic, if possible. In all cases, the injury or illness must be reported to one of the two locations immediately. The student must sign in the log book each time he/she enters the Training Room for treatment and should ask the Training Room staff for assistance upon entering. At no time is the student to help himself/herself to Training Room equipment or supplies. The Training Room is a designated medical treatment area and should be treated as such. Only students needing its services should be present. No athletic equipment or shoes should be brought into the Training Room and proper attire is required. Prescriptions Prescriptions, for intercollegiate athletically related injuries only, authorized by the Sports Medicine Clinic during approved and coach-supervised practices and competitions for both the championship season and nonchampionship seasons as designated by the Director of Athletics and Recreation Services are filled at the Student Health Dispensary. To receive an authorized prescription, the student will be given a “pink” Sports Medicine prescription slip which he/she should present to the Student Health Dispensary for filling. If the student receives a prescription from a specialist (a physician other than one associated with the Sports Medicine Clinic or Student Health Services staff) to whom they have been referred by the Sports Medicine Clinic, it will be necessary for the student to bring that prescription to the Sports Medicine Clinic for it to be recorded on the “pink” prescription slip for filling. No prescriptions will be paid for unless this procedure is followed. If, after a prescription slip has been issued by an outside physician, the student finds the Sports Medicine Clinic closed, the student should take the prescription to the nurse on duty at the Laurel Hall Student Health Service. The student will receive a temporary supply of medication. The Division of Athletics and Recreation Services will not pay for a nonpink prescription filled at any pharmacy. If subsequent appointments are recommended by the referral physician, you must receive approval for each visit by the Sports Medicine Clinic. d. After referral appointments, you must return to the Sports Medicine Clinic for follow-up. Note: In the unlikely event the Sports Medicine Physician cannot be reached, contact the Student Health Center. Do not schedule an appointment with a non-University physician without contacting the Sports Medicine Clinic, as the expenses will not be covered by Athletics/Recreation Services medical insurance. In a life-threatening situation, seek emergency help immediately. 3. It is the responsibility of the student-athlete/cheer unit member to fulfill the requirements of the primary insurance. This includes, but is not limited to the requirement for surgery to be performed in the home state.
