APPLICATION FOR PROMOTION INSTRUCTIONS
INSTRUCTIONS:
1. Complete the Application for Promotion via Qualtrics survey
(https://uwstout.qualtrics.com/SE/?SID=SV_74aQXivCxYYAyd7).
a. You must answer all survey questions with the exceptions of the last two items,
which will be completed by your immediate supervisor after you have printed
your survey results.
b. After answering all the questions, with the exception of the last two items, submit
the survey and print your results.
c. If you have missed a question, the survey will alert you of the missed question.
You must answer the question before you will be permitted to submit the survey.
If you submit your survey, but fail to answer a question (or questions), you will
see a screen like this:
d. After submitting the survey, the screen will process for about 5 seconds and then
your survey answers will appear on the screen. You may print that screen or
export the survey as a PDF and then print the PDF. To export the survey results as
a PDF, click the Adobe PDF icon in the upper right hand corner of your screen.

e. You may complete the survey multiple times if you wish to change your answers.
f. Print the application and submit an original and eight copies to your immediate
supervisor by the deadline published in the schedule of Promotion Procedures for
this year. The immediate supervisor must sign the application. Electronic
signatures will not be accepted.
2. Along with the application, include a document, of no more than five pages, detailing
your qualifications for promotion. This document is a record of your professional and
scholarly activities reflecting sustained excellence of performance within one's job
description. Print an original and eight copies of the document detailing your
qualification for promotion.
a. Document Format
i. The document format should include the following sections: introduction,
teaching, research, service, faculty development, and pertinent educational
experience.
ii. The document must meet the following format: five or fewer typewritten
pages, one side only, standard 1-inch margins, and a minimum Calibri 10
point font.
b. Document Content
i. This information must describe only activities during the preceding five
years except for publications which may be drawn from the last seven
years. Full professor candidates may use significant activities prior to
seven years. Dates shall accompany all supporting data. (See Chapter
IIIb of FASLA [Faculty/Academic Staff Limited Appointees Handbook])

ii. Policy No. 85-40, "Definition of Teaching, Research and Service,"
provides the university’s general definition of teaching, research, and
service (http://www.uwstout.edu/parq/upload/85-40.pdf)
iii. A copy of your department’s definition of research may be included in the
application. That statement DOES NOT count toward the five page limit.
3. Complete the first two lines of the Summary of Promotion Action form, signing your
name on the line that reads “Faculty Member.”

form and include eight copies with your promotion application.
4. The combined promotion recommendations for split assignments shall be forwarded to
the appropriate committee as a single application.
