2016-17 Optional Computer Purchase (715)
I want to be considered for additional loan eligibility (up to $1,000) for the purchase of a
computer for the 2016-17 academic year. I have attached a receipt, lay-away slip, or price
quote dated between July 1, 2016 and June 30, 2017 which contains the following items:

I understand the following:
o If I have borrowed the maximum annual federal loan limit, I may have to apply for a
private alternative loan or my parents may have to apply for a Parent PLUS Loan. (See
information about alternative and PLUS Loans in the Loan section of www.wiu.edu/fa).
o If a loan is processed, loan funds will be applied to my university account.
o If I owe a balance on my university account, I may not receive a refund for computer
costs.
o I understand I will be contacted through my WIU e-mail account after review is
complete.
