TITLE 3400 - FOREST INSECT AND DISEASE MANAGEMENT
3455 - REPORTS. Accomplishment and Expenditure Reports. National Forests
and States which have conducted suppression and prevention projects during the
preceding fiscal year should submit the following reports to the Regional office by
October 1.
1. Form 3400-5, Forest Insect and Disease Management Accomplishment
Report. All insect and disease project accomplishments financed from FPM funds,
which are conducted by National Forest and States, are to be reported on Form
3400-5. in addition, a brief narrative accomplishment posttreatment report

for all suppression and prevention projects.
Since most projects in the Region involve mountain pine beetle the narrative outline
below serves as an example for these projects. Narratives for other types of projects
should cover similar information as appropriate.
The brief narrative should summarize for projects and for major units within
projects the applicable items of the following:
Total acreage of suppression (by treatment method)
Total number of infested trees treated (by treatment method)
Total number infested trees not treated (trees spotted and not treated plus
infested trees which were not spotted in time to be treated)
Total cost (all funds) per treated tree (by treatment method)
Sampling results verifying treatment effectiveness - Post- treatment
Evaluation
Treatment accomplishments for mountain pine beetle prevention are reported
in acres of susceptible host type that have been completed within the project
area.
Posttreatment Standards
All nontreated infested trees must be located, probable cause for no treatment
established, and the percentage of treatment omission calculated.

Treated Trees
For infested trees that are chemically, or fell - and burned-treated, verify treatment
effectiveness on a sample of at least 30 trees or 5 percent of the total treated trees
distributed throughout a project or for major units within large projects.
For projects using pesticides, refer also to FSM 2140 for pertinent direction and
requirements for posttreatment evaluations.
For infested trees removed and processed, verify that removal of all cut trees did
occur on a sample of at least 10 percent of the infestation groups distributed
throughout each project or by major units within large projects.
