61 - QUALIFICATION
61.2 – Identification Card or Document
The Superior National Forest will use the Operator Identification Card (ID) card to identify those
employees authorized to operate specific pieces of specilized equipment. Specialized equipment
includes: ATVs, snowmobiles, trailer towing, motorized boats, tractors, skid loaders, Otters,
forklifts, graders, trail bikes, any fire equipment equipped with lights and/or sirens, motorcycles,
15-passenger vans, and any other motorized-wheeled equipment. Defensive driving training
requirements will be tracked for those employees issued Operator ID cards (i.e., operator ID
cards will expire on the 3rd anniversary of employees defensive driving training date).
NOTE: Employees are authorized to drive automobiles (sedans, pickups, etc.) solely
based on their valid state-issued drivers license and will not be required to have an issued
Operator ID card to drive vehicles under 26,000 Gross Vehicle Weight (except for Fire
program engines and 15-passenger vans).
Commercial Drivers License (CDL) requirements apply to all employees. Individual employees
must maintain the appropriate CDL required for the type of vehicle/cargo being hauled and
identify themselves to the Forest Safety Manager for inclusion in the random drug testing
program.
Specific requirements for the Forest’s Operator ID program include:
1. Documentation:
a. Operator ID cards will be issued by unit fleet representatives on form OF-346,
United States Government Motor Vehicle Operator’s Indeitification Card.
b. Unit Fleet Representatives will track issued Operator ID cards on a table (Exhibit
1), listed in card number order. This table will identify each driver and training
fulfillment dates for each type of specialist equipment and defensive driving
completion dates.
c. Operator ID Cards will be issued with an expiration date that matches the 3rd-year
anniversary of the employee’s Defensive Driving completion date. No forest
Operator ID card will be issued with an “indefinite” expiration date.
d. Unit Fleet Representatives will maintain a Driver Qualification History file on each
employee issued an Operator ID card. This file (filed under file code 7130-6) will
hold all documentation received that shows/proves the employee has met required
standards for operating an identified piece of specialized equipment (Exhibits 2 & 3).

OPERATORS
The Unit Fleet Representative will only include a piece of specialized equiment on
the employee’s Operator ID card that is supported by documentation attesting to the
employee meeting approved certification standards.

62 – TRAINING
62.1 – Training for Qualification
The Forest Safety Manager/Committee shall:
1. Establish training requirements/program for each type of specialized equipment.
2. Locate, schedule and notify employees of available specialized equipment training
programs.
3. Evaluate and certify Forest employees designated as instructors for specialized
equipment. Only certified instructors may “certify” employees to operate each specific type of
equipment.
