Chapter 30 – ACCIDENT INVESTIGATION AND REPORTING
32 – ACCIDENT NOTIFICATION
32.1 – Notification Criteria and Process Notification procedures in the event of serious injury or death
to a Forest Service employee or contractor will be as follows:
Immediately notify Forest Supervisor or acting and the District Ranger or acting. They will be
responsible for notifying the responsible local law enforcement official and the following in the order
listed:

In a serious injury case next-of-kin will not be notified until it is confirmed through the immediate
supervisor as to the name(s) of the employee or contractor involved. In death cases notification will
not be made until confirmation comes from the responsible official charged with making those
determinations. (ie. coroner or doctor).
The Forest Supervisor or the designated acting will handle notification of next-of-kin. Such
notification will be made in person where feasible and will be coordinated with local law enforcement
officials.
