Patient Information Sheet

Purpose

The University Health Services (UHS) is an accredited medical practice operated by Monash University. A broad range of general health services is offered to the University community with an emphasis on illness prevention and health education.

Attendance

The UHS is open continuously 9.00am to 5.00pm Monday to Friday. Appointments are encouraged and this reduces waiting time. Open access consultations are available. Longer consultations are available and may be arranged by prior appointment. Phone calls are taken in relation to recent consultations. Attendance is required for results and repeat prescriptions.

Home visits

Doctors from the UHS are available to make home visits in some special circumstances. Please discuss with your doctor.

Emergencies

Advice or attendance for on campus emergencies may be sought from the UHS by dialling Security on 333

General Practice Fees

Australian resident students and international students are bulk-billed

– in these situations no fee is raised to the individual attending. Fees may be charged directly to the individual for some routine medical examinations. UHS also has a set of private fees. Payment is made at the time of consultation. Members of the University receive a discount service. A schedule of private fees can be obtained by speaking with our receptionists.

Travel medicine

The UHS offers a comprehensive range of travel information. Travel advice should be sought at least 6 weeks before departure. A travel questionnaire needs to be completed prior to consultation with a doctor to ensure that appropriate vaccinations and optimal advice are given. Immunisation

All routine childhood, occupational and travel immunisations are available.

Sexual health

The UHS provides advice in all areas of sexual health, including advice on STDs, contraception, pregnancy and the provision of smear tests. All of the attending practitioners are available to give non-judgemental advice.

Medicals

Medicals are available for occupational and licencing needs.

Counselling and Psychology Services

Practitioners are available for counselling and clinical psychology consultations. Please ask at reception for further information

Dental Services – Clayton Campus only

Procedures

Minor procedures are performed including treatment for ingrown toenails, plantar warts, skin biopsy and wound suturing.

Reminders/Recalls

UHS has a system for follow up and recall of patients with abnormal test and imaging results and for this reason it is important for us to have current contact details for you. For details please discuss with your G.P.

We also offer you the opportunity to be placed on our reminder system for management of routine health issues.

After hours

Should you require care outside our hours please contact the duty doctor on 132 660. (This service will be bulk billed).

Privacy

The privacy of health care services is prescribed by State Legislation in the Health Records Act 2001 and the Federal Information Privacy Act 2000. The UHS has a Privacy Policy that is available at the web site or upon request. This practice is committed to maintaining the confidentiality of your personal health information. Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff

Your responsibilities

Provision of health care involves a responsible partnership between the UHS and you. The UHS endeavours to assist with all matters related to your health or illness. It is your responsibility to ensure that you obtain the results of tests by making follow up appointments, with the UHS or with referred specialists.

Telephone Access

Practitioners in the UHS may be contacted during normal business hours. If the Practitioner is consulting, a message will be intra-mailed them so they can return your call. Your call will always be put through to the

Complaints

In the case of dissatisfaction, it is appropriate that the matter be discussed with the Service provider or
