Grade Posting Request
If you want your grades posted on my web page during the semester then you need to print this form, fill in
the necessary information, and return it to me.
All grades posted on my web page will be posted by a 5 symbol alpha/numeric code. I will not distinguish
between upper and lower case letters. Your code must have exactly 5 symbols or your grade will not be posted.
Your code also CAN NOT be any part of your Social Security Number.
Reminder: If you give your code to your friends, classmates, or parents, they will be able to see
how you are doing in the course. They will see what you made on every quiz, homework, and
exam.
At any time you can decide that you don’t want your grades posted. Inform me by e-mail or talk to me in
class and when I update the grades I will remove you from the list. If you decide you want a new code then
fill out and turn in another form.
Note: In the event of duplicate codes, then I will not post the grades of either person. If I can not read your
code, your grades will not be posted.

I have read the above and I understand that the posting of my grades is not secure. I also understand that
anybody with an Internet connection will be able to see the posted grades. I understand that I can at any time
request that my grades be removed from the web or that I can change my code.
