I.E.P and 504 are considered insufficient forms of documentation. Must include testing on which accommodations are based. Provided by school at no cost to student Evaluations are at student expense Evaluations provided at prescribed intervals Re-evaluations are uncommon after initial approval, however certain circumstances may warrant additional documentation. Student/Parent Responsibility Student Responsibility Identification by parent and/or teachers. Self-identified and disclosed. Primary responsibility for accommodation belongs to school. Primary responsibility for accommodation belongs to student Parent has access to student records and participates in accommodation process. Parent has no access to records and does not participate in accommodation process. Teacher/Instructor Role Teacher/Instructor Role Modification of curriculum and alteration of pace of assignments possible. Testing usually weekly with graded assignments and mid-term and final examination. Frequency of testing and assignments vary and are often noted on course syllabi without reminders. Grading may be modified based on the quality of the curriculum. Grades reflect submitted work. Least restrictive environment option Least restrictive accommodation decision (instructor) No modification of essential criteria or functions allowed. The length of the academic term and essential criteria of course/program determine pace. Procedures • Everyone must self-disclose to the Department of Disability Services (DDS) • Complete Written Request for Accommodations using forms found on our website • Have Supporting Documentation sent to Department of Disability Services from diagnosing professional • Make an appointment with the DDS to review the findings and learn of expectations. Documentation Procedures • DDS will Review Forms and forward or deliver written verification. • Detailed information concerning a diagnosis is only shared with DDS and not the instructor. • Any information including a diagnosis or details pertaining to a diagnosis should be directed to Disability Services only. Documentation Procedures Responsibilities – Timely and appropriate disclosures by those needing accommodations. – Written request for accommodations using forms on WSSU website. WSSU does not provide funds for transportation, testing for documentation, tuition, textbooks, hearing aids or any individually prescribed devices. Veterans may also be eligible for services. All accommodations are coordinated through the WSSU Department of Disability Services. PARKING • All students and employees, whether full-time or part time, must register vehicles driven on campus. A vehicle is considered registered ONLY when the permit is displayed. • Persons with disabilities that have a state handicap placard in their name will be eligible to purchase a WSSU accessible parking permit.
