insufficient forms of documentation.

• Have Supporting Documentation sent to
Department of Disability Services from diagnosing
professional
• Make an appointment with the DDS to review the
findings and learn of expectations.
Documentation Procedures
• DDS will Review Forms and forward or deliver
written verification.
• Detailed information concerning a diagnosis is
only shared with DDS and not the instructor.
• Any information including a diagnosis or details
pertaining to a diagnosis should be directed to
Disability Services only.

– Timely and appropriate disclosures by those
needing accommodations.
– Written request for accommodations using
forms on WSSU website.
WSSU does not provide funds for
transportation, testing for documentation,
tuition, textbooks, hearing aids or any
individually prescribed devices.

Accessibility Statement 2014-15
Students with ADA-documented physical, sensory, psychological, emotional or
medical impairments may be eligible for reasonable accommodations. Veterans may
also be eligible for services. All accommodations are coordinated through the WSSU
Department of Disability Services. You may contact the Department of Disability
Services by email at dds@wssu.edu.
If you have registered in previous semester, continuation is not automatic. Action is
required on your part. Please contact dds@wssu.edu to continue your
accommodations.
Accommodations are not retroactive.
PARKING
• All students and employees, whether full-time or
part time, must register vehicles driven on
campus. A vehicle is considered registered ONLY
when the permit is displayed.
• Persons with disabilities that have a state
handicap placard in their name will be eligible to
purchase a WSSU accessible parking permit.
