COLLEGE APPLICATION PROCEDURES
2010-2011
PLEASE READ THE FOLLOWING PROCEDURES THOROUGHLY. IF YOU HAVE
ANY QUESTIONS OR REQUIRE SPECIFIC ASSISTANCE, PLEASE CONTACT
ANA, GIRELLE, OR MAURA. ONCE YOU AND YOUR PARENT/GUARDIAN HAVE
READ AND FULLY UNDERSTOOD THE PROCEDURES, PLEASE SIGN THE
AGREEMENT AT THE END OF THE DOCUMENT. SUBMIT THE AGREEMENT
TO THE COLLEGE OFFICE.
TRANSCRIPT EVALUATION
1.
Review your transcript to make sure you are on track for graduation. If you have any questions about
this, please make an appointment to meet with Victor or Ana.
2.
CHECK YOUR TRANSCRIPT to ensure that it is accurate. If you find errors, please make notes on
the transcript and give back to Pierina IMMEDIATELY.

If you have any questions about a grade, speak to your teacher. If resolving the issue requires a grade
change:

Fill out a “Change of Grade Form” on the College Bound Wiki

Make a copy for yourself and give Pierina the original.
All changes and corrections must be completed by Friday, October 22, 2010. The College Office is
not responsible for transcripts that contain errors.
TIMELINE AND DEADLINES
1.
Submit to the College Office any forms that must be filled out for colleges or scholarship providers two
weeks prior to the application deadline. Student information sections must be complete. Indicate
whether or not you also need a copy of your SAT/SATII/ACT scores.
2.
If you are applying early admission/early action, you must notify the College Office and submit forms
that the College Office needs to complete by Wednesday, October 20, 2010. Remember: Early
Admission/Early Action deadlines are generally November 1st or November 15th. YOU ARE
RESPONSIBLE FOR KEEPING TRACK OF YOUR DEADLINES.

Requests for applications with a preferred or deadline date of December 1st must be submitted to the
College Office by Monday, November 15th 2010.

Requests for applications due January 1st or January 15th, 2011, must be submitted by Wednesday,
December 1, 2010. Remember, holiday mail takes longer and we will be on break from December 24,
2010, to January 3, 2011. The key word here is BREAK.

Online applications are preferred by many colleges. Some allow you to apply for free or at a reduced rate when
you submit the online application. This year the College Office will submit forms and transcripts electronically
whenever possible. The Secondary School Report/Counselor Recommendation paper forms must be
submitted to the College Office even if the application and supporting documentation will be submitted
electronically. Fill out your section and submit these forms for completion. The College Office requires at
least two weeks to complete any paperwork. A notification from the Common or Universal Applications
does not constitute a formal request.

CITY UNIVERSITY OF NEW YORK (CUNY)

Go to “Apply to CUNY.”
You can apply to six schools with one single application. The total cost is $65.00 (sixty five dollars)
regardless of the number of schools to which you apply.

DO NOT INPUT YOUR OSIS NUMBER. This is very important. Our database is not connected
to the Department of Education’s (DOE) system. Only courses taken during summer school or
Regents Exams scores will come up if the University Application Processing Center (UAPC)
accesses your DOE’s records. YOUR APPLICATION WILL BE DELAYED.

You may pay with Visa or MasterCard. When you are done, print an “Application Summary
Package.” Submit the “High School Transcript Request Letter” and your receipt to the College
Office so we can send it with an official transcript and your SAT scores to UAPC.
Alternatively, you may pay by check or money order. If you select this option, make sure to print
your payment ticket. Your application will not be processed until your payment is received. The
check or money order should be made out to CUNY UAPC. The cost of the application is $65.00
(sixty five dollars). Bring the check or money order, the payment ticket, and the “High School
Transcript Request Letter” to the College Office. We will mail them to UAPC along with your
official transcript and SAT scores.
If you are eligible to receive a FEE WAIVER, please make sure to check your email regularly. Ana
will send an email letting you know that these have arrived. It is your responsibility to make sure
that Ana has your email address. Email her at afallariff@gmail.com if you are not on her
email list. CUNY gives our school a very limited number of fee waivers. Last year we only
received five. These are distributed on a first come, first serve basis. If you receive a fee waiver,
follow the same procedure outlined when paying by check or money order.
When is the CUNY application deadline?




February 1 is the priority deadline for the fall of 2011. After this date, applications are accepted on a
first come, first serve basis. WHAT DOES THIS MEAN? Any applications received after this date will
only be considered if space is available. Your application will not be processed until UAPC receives a
completed application, payment, official transcript, SAT scores, and any other supporting
documentation.
Please note: CUNY operates on a rolling admission basis; therefore, colleges and programs may close
before the deadline.
The College Office strongly encourages students to file the CUNY application as early as possible,
preferably before November 1st, 2010.

Visit www.suny.edu. Read the “Which SUNY Are You” section thoroughly
(http://www.suny.edu/student/). Familiarize yourself with the programs offered and with the admissions
procedures.

Apply online. You can find the application and other required forms at
http://www.suny.edu/Student/forms.cfm? Print and fill them out before inputting the information online.

Once you are familiar with the programs, access the application by going to the “Apply” section.
Download and fill out the “Online Application Worksheet” http://www.suny.edu/student/oas_content/2011_suny_app_worksheet.pdf

Fifty-one out of sixty-four SUNY campuses accept the SUNY Common Application. You can access
the list of non-participating campuses in the “Apply” section of the website. If the campus to which you
are applying does not accept the SUNY Common Application, make sure to request or download the
appropriate forms.
o The application fee is $50.00 per SUNY school. So, if you are applying to four schools, the
total payment will be $200.00.
o If you are eligible for a fee waiver, select “Mail-in-Payment” in the “Payment Type” section.
You can download a “Fee Waiver Request” form at
http://www.suny.edu/student/downloads/Pdf/2011_fee_waiver.pdf. The form gives you specific
eligibility information. You and the head of household in your family must fill out and sign the
form before it is processed by the College Office.
o After reviewing your application and submitting it, print out the “Online School Counselor
Form” - http://www.suny.edu/student/downloads/Pdf/2011_school_counselor_form.pdf. Submit
a completed form to the College Office. Make sure to submit a typed list of the individual
colleges and the addresses to which the supporting documentation should be sent. If you are
paying by check or money order, print out the invoice and bring it and the transmittal to the
College Office. The check/money order should be made out to “SUNY ASC." We will mail the
transmittal form, the invoice, and your payment to the Application Services Center. We will also
mail the transcripts and copies of your SAT scores directly to the individual campuses.
o Visit the “Supplemental Forms” section to check if the campus/es to which you are applying
require supplements. If they do, download and complete them. Sometimes you will receive an
email with the supplement as an attachment. It is recommended that you submit these within
fourteen days after you submit the application.
o Some colleges require additional information, letters of recommendation, or essays. Visit
individual school’s websites to inquire.
o Admission decisions are only made after all required documentation is received.
o If you are applying through the Education Opportunity Program (EOP), use the SUNY common
application. You can apply to up to four schools for free. Please make sure that you fit the
financial and academic requirements for the schools to which you are applying. If you do not
meet the requirements, you will have to pay the application fee.

Many of the SUNY schools use the “Common Application” (see below for instructions). Restrictions
apply if you need a SUNY fee waiver or if you are applying to the Education Opportunity Program
(EOP).

When is the SUNY deadline?


SUNY recommends that you submit your application by November 1 to better your chances for
financial aid, campus housing, and consideration for your major of choice.
Some colleges have specific application deadlines for certain programs of study. Make sure you know
what these are—do not miss any deadlines.

Some colleges offer Early Decision or Early Action programs. Check the school’s websites for specific
application deadlines.
ALL OTHER APPLICATIONS
1.
Determine if the school accepts the Common Application (www.commonapp.org), the Universal
College Application (https://www.universalcollegeapp.com/), Edu, Inc. Common Black College
Application (http://www.eduinconline.com/) or if it has its own application.
2.
Print a copy of the application and fill it out before doing so online. This will reduce errors.
3.
Complete all student portions of the application.
4.
Submit all forms that the College Office must complete. These may include a Secondary or High
School Report, or an Advisor/Counselor Evaluation. Be sure to fill out the student part of the forms.
5.
If the schools require SAT I/SAT II/ACT scores, find out if they accept our copy of the scores or a
transcript that lists your scores. If they do not, request that the CollegeBoard and/or ACT send them
directly to the college.
All Applications
 Submit completed College Application Form (See College Bound Wiki).
 The College Office must review your applications with you before they
are submitted. Please make an appointment with Girelle, Maura or Ana.
TEACHER EVALUATIONS/RECOMMENDATIONS
Think hard about which teachers you will ask for a letter of recommendation. Many colleges/universities
require at least two letters in addition to the counselor’s report. Make sure to read the required letters section
thoroughly: schools/programs may require you to submit recommendations from teachers in specific
disciplines. You must request the letters of recommendation at least three weeks before the date they are due.
Use the “Teacher Recommendation Request Form” available on the College Bound Wiki. When you list the
colleges/universities, indicate the deadline information. Please fill out your information before submitting the
forms to the teachers. You might want to give them two copies of the forms just in case. Maintain good
relationships with your teachers. They are not required to write letters of recommendation.
Your School Report/Counselor Evaluation will be based on the following factors: your responses in the
“Questions for College Counselor Recommendation Form” (see College Bound Wiki); Maura’s, Girelle’s and
Ana’s direct observations, and interviews with your teachers and other school personnel. Requests for School
Reports/Counselor Evaluations must be submitted two weeks before the application deadline.
TIPS AND REMINDERS

Purchase an expandable pocket folder. Dedicate two pockets to each school to which you are applying,
one for the admissions application and supporting documentation and another for the financial aid
application and documentation. Label each pocket.

Buy a wall calendar. Note your college/financial aid/scholarship deadlines; you can color-code them if
you wish. Place it in a central location in your home so everyone knows your deadlines.
READ THE APPLICATION INSTRUCTIONS THOROUGHLY. Familiarize yourself with the
application.
Complete a draft of your application before filling out the original. Do a draft of online applications as
well: download a copy and complete the forms before entering the information online. This minimizes
mistakes.
Check and double check deadlines and make a list of all required supporting documentation. Be sure to
check off items as you put together the application package. Incomplete applications will not be
processed by the colleges.
If the schools require SAT/SATII/ACT scores, find out if they accept the scores on our transcript or a
copy of the report we receive. If not, you need to request that the scores be sent directly from the testing
service.
Be sure to put your identifying information on all documents you are sending. Some schools require
you to write your name and social security number on every page of your essay. If paying by check,
write your name and social security or application number on the memo.
Be sure to send your payment or, if you qualify, a fee waiver.
Proofread your essay and your short questions. Give yourself plenty of time to fill out the application.
If you wait until the last minute, you might make mistakes or omit important information.
Be neat. A messy, unreadable application does not represent the applicant well.
Sign the application. If the application requires a parent/guardian signature, be sure that one of them
signs.
Make copies of all applications and all supporting documentation for your records.
Contact the college or university to make sure they have received all parts of the application. It’s your
responsibility to submit a completed application; it is not the college’s or university’s obligation to
inform you of missing parts.
The College Office welcomes the opportunity to support you through the college application process. YOU are
responsible for meeting college and scholarship deadlines and for making sure that colleges have received all
necessary forms and supporting documentation. Be sure to maintain contact with the colleges to ensure that
your application is on track.
