Create a New User Account and Login Change your Password To Create a Driver's Education Application and The Submission Process. 4 Create a New User Account. 1. 2. Creating A New User Account Go to the website one of two ways: • www.egohs.org or • www.gahighwaysafety. org and click on eGOHS Icon to begin the login process. Click the “Create a new use account” link. 5 Add Contact Information Creating A New User Account Register Contact. 1. Complete your contact information, but leave the Agency for last. 2. Click the Find Agency button and scroll through the list, select your school. 3. Make sure you include your email address and all areas with an asterisk*. 4. Click Register. Note: you can click at the top or bottom. 6 Search for Agency Creating A New User Account 1. If Agency/School information does not populate, enter the Agency’s Name, Address, City, State, Zip Code and County. 2. Use the dropdown box to identify the Category for Driver's Education. • Schools K-12 3. Click on Search. 7 Search for Agency Creating A New User Account 1. If your school is not listed after you have clicked on the Search button: 2. Search Options • School Name, or, • High School, or • County.. 3. Enter the information Listed. 4. Click on the No Agency Found. Note: Applicants are not able to delete duplicate entries, but should report it to the GOHS Help Desk. 8 Confirm User Registration Creating A New User Account 1. Your Agency Contact Information will show up in the Register Contact Section. Salutation: 2. Your Username will automatically populate in the Confirm Registration Section. Use dropdown box and include: Mr., Ms., Mrs., Officer 3. Put a password in twice (2x). 4. Click the Confirm button. 9 Confirm User Registration Creating A New Application 1. After you have registered, an eGOHS Administrator will release your “Security Level”. 2. You will receive an email confirming that you are now able to use the eGOHS System. (This may take up to 24/48 hours.) 10 Login 1. 2. Creating A New User Account Go to the website one of two ways: • www.egohs.org or • www.gahighwaysafety. org and click on eGOHS Icon to begin the login process. Login, using your eGOHS Username and Password. • Your username is typically your first initial and last name in lower case all together. • The password is the one you created upon login. If you didn’t create one, contact the GOHS Help Desk and has it reset. 11 Initiating An Application Creating A New Application 1. Use the dropdown window an click on Drivers Education 2009. 2. Click on Apply for a New Grant. 3. Answer question: Are you sure you want to create a new Application? 4. Click “OK. 5. The screen will refresh and take you directly to the Components Section. 12 Application Program Components Creating A New Application 1. Identify your Program Type: Driver Education. 2. Name (Reference only) section. It’s okay to put you Program Title or School Name. 3. SAVE. Note: If you skip this section and Return to Application Menu you will get the following screen. 13 Components Creating A New Application 1. If you skipped the Components section your Application Menu will look like this. 2. The Program Components must be answered and then the Application Forms Section will be available. 3. Click on Program Components. 14 Program Component Service Areas Benefiting Creating A New Application 1. Service Area Type 1. Metro Statistical Area is the default. 2. Go to the County Section. 3. Scroll through and highlight your County, then click Add. 4. Click the second tab: Representative Districts. 15 Program Component Service Areas Benefiting Creating A New Application 1. When the screen refreshes it shows the County in the lower section. 2. Now click on the Representative Districts tab. Review the Agency Information and make sure it is accurate and all the information is filled in. 2. Click on either Edit button to open to document. 23 View Agency Information Creating A New Application 1. Verify and correct the Federal Employer ID Number (it may have a default 99999 in it. 2. Correct your Federal Employer I.D. Number (obtain from the Bookkeeper or Finance Dept). 3. Type in the changes. 4. Save when completed. 5. Continue to scroll down the page. 24 View Agency Information Creating A New Application Agency Vendor Codes 1. Do Not Use, GOHS will be completed this section for you. 25 Agency Contact Descriptions (Security Levels) • Security Levels – Agency Authorized Official (AO) – Highest level in Agency/Jurisdiction (i.e. Superintendent, Principal, etc.) – Able to review and Submit the Application – Sign off on claim forms (optional with Electronic Signatures) – Project Director (PD) • Agency Administrator / Project Director - App • Grant Administrator – Grant – Able to Create/Initiate: Application, MAR, Claims, Amendments and Submit MAR, Claims and Amendments – Financial Officer (FO) • Agency Staff / Fiscal Officer - App • Financial Officer – Grant – Able to Create Claims and work on them. – Alternate Signer (AO designee) – Able to View Application / Grant only (i.e. Principal, Assistant Principal, etc.) – Alternate will sign on behalf of the Authorized Official (AO) – AO, PD, FO and Alternate Signer cannot be the same person, unless special permission given by GOHS. 26 View Agency Information Creating A New Application Agency Contacts 1. Complete the questions on the page: • Are funds being sought from other sources? NO • Does your jurisdiction receive any other federal funds from other sources? Yes • If so, how much? Name the amount. • When is your Audit Period? 07/01/2008 – 06/30/2009; please use the correct date format as shown. • Save and click Next. Budget Information is populated when it is entered and the Cost Category Summary is Saved. 3. Close PDF Screen 4. Click Next or Return to Application Menu. 40 Application Components Creating A New Application All submitted grant applications must include the following completed sections: • Problem Identification (describe the teen driving problem in the county or state) • Program Assessment (status of driver education in the community, school) • Project Objectives (what is planned to be accomplished) • Proposed Activities (how will the objectives be achieved) • Implementation Schedule with Milestone Chart (timeline for significant events) • Evaluation Plan (methods and collections for proving success) • Media Plan (grant and project progress to the community) • Self-Sufficiency Assurance (ability to sustain the program after funding ends) • Resource Requirements (describe the significant resources sought) • Itemized Budget (See Funding Options I, II, and III for typical eligible resources and aids) 41 Problem Identification Creating A New Application 1. Follow the Instructions on the page. 2. Suggestion to follow. 3. SAVE and click Next. 42 Problem Identification Creating A New Application 1. This section is populated (pulled) directly from the Problem Identification Form after it is Saved. 2. Close this screen and click Next. 43 Document Attachment Creating A New Application 1. Use the Document Title to give your document a Relevant Name. 2. Put cursor inside the Browse section. 3. Click on the Browse button and located your document to be attached and highlight it. 4. Click Open and the document attaches to your application. 5. Save and it will show as a link. 6. Click Next. 44 Stop and Start Creating A New Application If you stopped here you can do the following to get back to where you were. • Logged into eGOHS with same Username and Password. • Expanded Tree on left by clicking on it. • Click on the Proposal in Process Application you were working on. • Note the check mark where you left off. You can also see if you skipped a section (those not checked). 45 Program Assessment Creating A New Application 1. Follow instructions and Suggestions. 2. This page should include a detailed description of your current and/or previous highway safety efforts. Be sure to indicate what has or has not worked in the past. 3. Are the activities phased into the project in proper order? Are quantifiable measures being collected? Is a progress report planned? Close screen and go to Application Menu. 61 Application Menu • Creating A New Application Returning to Application Menu allows you to see the forms you have already worked with, as well as the forms that need to be completed. Budget Section • If you have Personnel Services, click on Personnel Service Detail – Position; if not, click on the Cost Category Detail and select a category where you have expenses. 62 Creating A New Application Budget Detail Pages and Summary The Cost Category Summary recaps all projected budget items in your application. The Budget for Driver's Education Programs Should Not Exceed $140,000.00. 63 Personnel Service Detail - Position Creating A New Application 1. Position/Title (Staff 1, Staff 3, Supervisor 1). 2. Pay Code (use dropdown menu) 3. Identify Hours Per Week to work 4. Pay Rate. 5. Percent of Time 6. Project Salary 7. Add for each person. 8. Edit, Clear or Delete if you need to make changes. 9. Each person’s Name should be different (Avoid Staff Worker, Staff Worker, Staff Worker. This method would become confusing when processing Claims. 3. Project Services section populates after you have entered the Benefits information and Saved the page. 2. This category applies to all persons entered in the Personnel Services Detail Position Section. If you have no “Other” Benefits, leave that section Blank, don’t enter N/A and 0. • SAVE and click Next. 66 Personnel Service Detail - Benefits Creating A New Application 1. Review your work. 2. List every Pay Period for that Pay Code. • Hourly, Weekly, Biweekly, Semi Monthly, Monthly. • These weeks should coincide with the grant cycle. 3. Add Additional Pay Codes as needed. 4. Follow the instructions as indicated. • Click on the View PDF button to review and print the Grant Terms and Conditions. 2. Follow the instructions as indicated: • Check the box to indicated if the individual is bonded. • Enter Alternate Signer (optional). 2. There are two sections in which to check for errors. 2. Click on Errors button to see all errors in the Application to be fixed. • The errors in the forms will show there as well. 90 Check Application for Errors 1. Review before submitting. 2. After Returning to the Application Menu, scroll down to Application PDF. 2. Click on Application PDF to download your completed application. 3. Print Application. 4. Review before submitting. 92 Submit Application Creating A New Application 1. The Agency Authorized Official (Superintendent / Principal) will have the ability to Submit Application. Application Deadline: TBD. Read and Print your Grant Terms and Conditions.
