Survey Monkey: Instructions
Please remember that you must keep any and all contact information, as well as all
recorded responses on a password protected laptop or desktop computer.
Please do NOT leave any printed documents that contain contact information or
recorded responses on a desk, table, or other area where it may be accessible to
others. Please keep all printed data in a locked drawer to which only you have
access.
Step 1: Open a new window in your Internet browser (Internet Explorer, Mozilla Firefox, Google
Chrome, Safari, etc.)
Step 2: Copy and paste the following link into the address bar:
https://www.surveymonkey.com/s/WJWDBFS
The survey should appear with Question 1 on your screen. You can click through and answer
the questions appropriately, and be sure to click “Done” once you have completed all questions.
Step 3: ***Clear your browsing history as soon as you have completed the survey. You will not be
able to submit another survey until you have cleared the browsing history on your computer.
Depending on the Internet browser that you are using, (Internet Explorer, Mozilla Firefox,
Google Chrome, Safari, etc.), the settings will be different for how to clear your browser
history. Below are the instructions for each one:
Internet Explorer:
1. Open Internet Explorer by clicking the Start button . In the search box, type Internet
Explorer, and then, in the list of results, click Internet Explorer.
2. Click the Tools button , point to Safety, and then click Delete browsing history. If you don't
want to delete the cookies and files associated with websites in your favorites list, select the
Preserve Favorites website data check box.
3. Select the check box next to each category of information you want to delete.
4. Click Delete.
Mozilla Firefox:
1. At the top of the Firefox window, click the Firefox button, go over to the History menu and
select Clear Recent History....
For Windows XP: At the top of the Firefox window, click the Tools menu and select Clear
Recent History....On the menu bar, click the Tools menu, and select Clear Recent History....At
the top of the Firefox window, click the Tools menu, and select Clear Recent History....
2. Select how much history you want to clear:

Click the drop-down menu next to Time range to clear to choose how much of your
history Firefox will clear.

Next, click the arrow next to Details to select exactly what information will get
cleared. Your choices are described in the What things are included in my history?
section above.
3. Finally, click the Clear Now button and the window will close and the items you've selected will
be cleared.
Google Chrome:
1. Click the Chrome menu
on the browser toolbar.
2. Select Tools.
3. Select Clear browsing data.
4. In the dialog that appears, select the "Clear browsing history" checkbox.
5. Use the menu at the top to select the amount of data you want to delete. Select beginning of
time to clear your entire browsing history.
6. Click Clear browsing data.
Safari:
1. Open up Safari.
2. Go to the History menu.
3. Choose 'Clear History' at the bottom of the pull down menu.
4. Repeat as needed.
Step 4: Repeat steps 2 & 3 for each new response
