These are the only forms that can be used. Forms due on July 1, 2015, for Fall Sports.
PHYSICALS
• Must be conducted by the student’s own physician/healthcare provider
• Forms must be returned to school nurse during school year
• During summer months, deliver completed forms to Main Office in Anthony
(East) Wing of PHS (box labeled “Sports Physicals”). Forms must be reviewed by
the school physician.
SPORTS PHYSICALS
Those who do not have a private healthcare provider or who
experience financial hardship may have their child examined by
the school physician or may request information from the nurse
regarding nearby Health Centers.
Physicals will be available @ PHS West Wing (Patton) Gym
•Boys = Monday, June 19, 8:00AM – 12 noon
•Girls = Wednesday, June 17, 1:00-5:00pm
