Role Specific Job Description and Person Specification Job Ref: Job Title: Senior Social Work Practitioner, Continuing Healthcare (CHC) Team Base: Continuing Healthcare Team Directorate: Health and Adult Services Grade: I Accountable to: Graham Bentley - Team Leader, Short Term Team Context and Summary of Role Purpose As a CHC Care and Support Coordinator, you will play a critical role in ensuring that Kingston Clinical Commissioning Group (KCCG) is offering personalised care packages to people who have been identified as eligible for CHC funding whilst achieving value for money in its commissioning activities. You will be part of a small CHC assessment team based within Your Healthcare but report, with regard to professional activity and development to Graham Bentley, Team Leader, Short Term within the Royal Borough of Kingston Upon Thames Adult Social Care. This role is a joint role and will ensure that both health and social care outcomes for individuals are achieved by working in partnership with both commissioners and care providers. Key aspects of this role will be: Involvement, where appropriate, in professional and timely CHC assessments for people identified as potentially requiring CHC funding; Involvement with the multi disciplinary team in CHC assessments and decisions; To identify services and coordinate a caseload of care and support packages with full involvement of the person, family and other relevant and professional stakeholders; To offer support and assistance to the CHC team of nurses where social care services may be required for an individual who has become eligible for CHC funding; To carry our reviews of packages of care in a timely manner to ensure value for money of commissioned services Undertake Adult Safeguarding investigations in line with present processes in ASC. Key Roles and responsibilities Assessments 1. Support all assessors in the completion of assessments and liaise with families as required. 2. Maintain a knowledge and awareness of legislation and departmental policies and procedures, and work within these. 3. Work co-operatively and in partnership, with other professionals across Council (e.g. Housing, Adult Services and voluntary sector organisations) either based in a multi-disciplinary team in a Health or other setting. Case Management and Care Coordination 4. Ensure information flows to the integrated commissioners and finance functions for the effective management of data to facilitate payment processes. 5. Arrange the care packages both in the community or in Nursing homes settings for those patients who are eligible for CHC funding, working in partnership with YHC CHC Nurses and KCCG and RBK Commissioners. 6. Work in partnership with service users and carers, using a person-centred approach, with the aim of maximising independence, well-being and choice. 7. Be responsible for undertaking and supporting a range of case work tasks for patients with complex health needs. 8. Provide full assessments of patient need, in conjunction with relevant professionals and family members, in order to decide with the service user on a course of action. 9. Create, organise and administer appropriate packages of care and liaise, regularly, with service providers in all settings to ensure that packages remain appropriate. 10. Carry out reviews, in conjunction with commissioners in both KCCG and RBK, accordingly. 11. Assist the KCCG Commissioning Manager to ensure that placements and care packages are of a high quality standard through performance management and quality assurance systems. 12. Develop, in conjunction with CHC nurses, person centred, outcome based support plans. 13. Undertake risk assessments, where appropriate, for both service users and the carers associated with them. 14. Commit to embedding and improving social work practices in CHC commissioning in line with CHC guidance. General 15. Keep both electronic and manual file records up to date in line with YHC/RBK and KCCG’s guidance. 16. Raise the profile of continuing healthcare amongst practitioners through communication and regular attendance at relevant meetings. 17. Complete reports as required. 18. Work with appropriate information technology and maintain effective administrative systems whilst maintaining records for workload management purposes. 19. Attend supervision sessions, undertake training and participate in team meetings and/or case conferences as required. 20. Carry out all tasks in accordance with equalities policy and guidance. 21. Be fully aware of the principles of safeguarding as they apply to vulnerable adults in relation to the work role. To ensure that the team manager is made aware and kept fully informed of any concerns which you may have in relation to safeguarding and/or child protection. 22. Undertake a variety of methods of professional intervention. Develop knowledge and skills through keeping abreast of national developments, research etc. Promote improvements in practice standards and develop and evaluate new methods of work. 23. Contribute as required by line manager to the training of other staff, locally and through programmes arranged by KCCG or RBK partners. Performance Management 24. Deliver services and support in accordance with agreed service standards. 25. Be responsible for own personal performance and development in accordance with KCCG, YHC and RBK policies. 26. Contribute to the creation and implementation of an overarching quality and assurance framework for both health and adult social care services across KCCG and RBK in relation to CHC.. 27. Contribute to the overall performance of the team. Additional Person Specification Requirements Factors Description Education Previous Experience Experience: Knowledge of the NHS and Community Care Act and other relevant legislation. Demonstrable understanding of the principles and application of excellent adult social care Demonstrable experience of creating effective working relationships with internal and external customers Strong literacy, numeracy, interpersonal and IT skills. Experience of a similar role in a Local Authority or Health setting . A minimum of 3 years experience as a qualified practitioner Knowledge , skills and aptitude Educated to degree level with appropriate professional qualification relevant to the role and specialism (e.g.- CSS/CQSW or DipSw Dip COT or Degree in Occupational Therapy) To have attained a relevant Post Qualifying award. E.g. Practice Teaching, PQ Award Understanding of the different roles within the service.
