Since 1985, financial support to 5 community health centers
for general operations

In 1993, as a response to Managed Care, health centers in
collaboration with the City of San Jose and County of Santa
Clara, formed Community Health Partnership

In 2001, joint development and implementation of the
Healthy Kids coverage program for children who do not
qualify for Medi-Cal (up to 300% FPL)*

In 2012, voters of Santa Clara County passed Measure A, a
one-eighth cent sales tax increase, which would raise
$50 million annually to bolster health and safety programs
*In 2014, this was raised to 400%FPL
