Background:
CNFA worldwide is dedicated to stimulate rural economic growth through supporting and empowering the
private sector. CNFA is successfully implementing projects focused on development of privately owned input
and machinery services, stimulating market driven approach, establishment of demonstration plots and
delivering training programs on production modern technologies, supporting and facilitating creation of market
linkages among producers, input suppliers and retailers, thus increasing the competitiveness of agricultural
sector.
Currently CNFA implements two projects in Georgia: USAID/Georgia’s Restoring Efficiency to Agriculture
Production (REAP), a $19.5 million program that is designed to increase income and employment in rural
areas by delivering firm-level investment and technical assistance to expand the operation of existing
smallholder farmers and rural enterprises; and the DANIDA/SDC funded the Rural Economic Development
(RED) Programme for the Southern Regions of Georgia that aims to enhance the efficiencies and effectiveness
of the potato and dairy value chains to improve household incomes and living conditions within the Kvemo
Kartli and Samtskhe-Javakheti regions.
CNFA-Georgia is soliciting quotations from eligible and responsible companies to provide health insurance
coverage for its employees. The insurance plan should include but not be limited to:

RFQ - Proqurement of Health Insurance for CNFA-Georgia Employees
A helath insurance company should agree to the following conditions:
In case when financing of medical service is anticipated via state or a referral program the Insurer takes
the responsibility to cover amounts payable by the Insured as a required co-payment, in accordance
with the terms of an Agreement.
In case of end prothesis, as well as surgeries with the use of other medical corrective devices, the Insured
retains the right to choose the brand/ model of such a medical device (devices).
It is anticipated that an insurance provider company will be selected and the contract will be awarded no later
than January 31, 2015.
Currently CNFA has 40 employees, of which approximetly half need a family package. The amount of
employees is subject to change. CNFA will inform the selected company of new employees and and in case of
employee leaving CNFA, the selected company will be notified and the individual will be removed from the
list.
CNFA has offices in Tbilisi, Kutaisi and Telavi, and also a Regionl Coordinator located in Ozurgeti.The
medical coverage and service provision must be available in all above locations.
The required coverage period will be February 1, 2015 – January 31, 2016

All correspondence and/or inquiries regarding this Request for Quotation must be in accordance with the
instructions contained in this RFQ. Failure to comply with the instructions described in this RFQ may lead to
disqualification of an offer from consideration.

OFFER DEADLINE
Offers must be received no later than 18:00, Tbilisi, Caucasus Standard Time, January 9, 2015. Late offers will
not be considered by REAP and must be sent by e-mail or delivered on a ciled envelope to the party listed
below on or before the designated offer receipt date and closing time.

QUESTIONS/CLARIFICATIONS
All questions and/or clarifications regarding this RFQ must be in writing and can be submitted to
procurement@georgiareap.org Questions and requests for clarification must be received by email no later
than 18:00, Tbilisi, Caucasus Standard Time, on January 5, 2015. Questions and requests for clarification—
and the responses thereto—that CNFA believes may be of interest to other offerors will be circulated to all
RFQ recipients who have indicated an interest in bidding.
Only the written answers issued by CNFA will be considered official and carry weight in the RFQ process and
subsequent evaluation. Any verbal information received from employees of CNFA or any other entity should
not be considered as an official response to any questions regarding this RFQ.
3.
ELIGIBILITY OF OFFERORS
In accordance with Federal Acquisition Regulation Clause 52.209-6, “Protecting the Government’s Interest
When Subcontracting with Contractors Debarred, Suspended, or Proposed for Debarment,” (SEP 2006), all
offerors must certify that they are not debarred, suspended, or proposed for debarment by the USG. CNFA
will not award a subcontract to any firm or firms’ principals who are debarred, suspended, or proposed for
debarment, or who proposes to do business with firms (including subcontractors) or firms’ principals who are
debarred, suspended, or proposed for debarment, in the performance of the requirement of this activity.
4.
QUOTATIONS
Price quotations should be expressed in Georgian Lari and all-inclusive of any associated costs.
Please, note that all of CNFA’s programs in Georgia are free from VAT and any other taxes. Offered prices
must not include VAT. Quotations should be written in English, and contain a following information:







Bidder’s full legal name, contact information and the name of the authorized representative;
Insurance plan;
List of clients;
List of medical providers’ network in Tbilisi, Kutaisi,Telavi and Ozurgeti
Detail description of the process of reimbursement of expenses and/or patient co-payment terms;
The duration of the validity of the quotation. The duration of validity of the quotation should be no less than 50
working days.
TERMS AND CONDITIONS OF THE RFQ
Issuance of this solicitation does not in any way obligate CNFA to award a subcontract, nor does it commit
CNFA-Georgia to pay for costs incurred in the preparation and submission of the offer. CNFA reserves the
right to make revisions to the content and order of the provisions in the actual purchase document prior to
execution by CNFA-Georgia and the selected awardee.
7.
BASIS FOR AWARD
The award will be made to a responsible offeror whose offer follows the RFQ instructions, meets the eligibility
requirements, and is determined via a trade-off analysis to be the best value based on application of the criteria
below. A Review Panel, appointed by the CNFA-Georgia, will evaluate quotations submitted by eligible firms
in accordance with the following:


by employees themselves)1
Responsiveness to cover requested services (40%) – Based on degree that the offeror meets items of
required services
Please note that if there are significant deficiencies regarding responsiveness to the requirements of this RFQ,
the offer may be deemed “non-responsive” and thereby disqualified from consideration.
8.
NEGOTIATIONS
Best offer quotations are requested. It is anticipated that awards will be made solely on the basis of these
original quotations. However, CNFA-Georgia reserves the right to conduct negotiations and/or request
clarifications prior to award.
10.
PAYMENT AND AWARD
The contract will be awarded to the offeror whose quotation represents the best value to CNFA’s ongoing
projects. The payment will be made through a contract between CNFA’s relevant project and the offeror on a
monthly bases.
Bidders are responsible for ensuring that their offers are received in accordance with the instructions stated
herein. Failure to adhere to the instructions described herein may lead to a quotation’s disqualification from
consideration.
The negotiated contract/agreement is a subject to CNFA’s terms and conditions.
Please note that in submitting a quotation, the bidder understands that USAID or DANIDA/SDC are not parties
to this RFQ. Therefore the bidder agrees that any protest hereunder must be presented in writing with full
explanations to CNFA for consideration. CNFA, at its sole discretion, will reach the final decision on the
protest for this procurement.
