You will receive a call from a Credit Union Representative with further payment options. Payments will be for a period of 12 months. Your loan will be life and disability
insured. For value received, I/we promise to pay jointly and severally the sum requested above and costs of the loan to Luminus Financial Services & Credit Union
Limited. For Valuable Consideration hereby assign, transfer and set over unto the Credit Union, (i) 20 per cent of all wages, (as defined in the Wages Act of Ontario),
but excluding any amount that an employer is required by law to deduct from any such wages and (ii) all other monies owing to me, or hereafter to become owing to
me, by any person, firm, corporation or entity by whom I am now or may be hereafter employed. AND I HEREBY AUTHORIZE AND DIRECT my employer or any
future employer to pay the said 20 per cent of all such wages and all such other monies to the Credit Union, and I hereby irrevocably authorize the Credit Union to
take all proceedings which may be proper and necessary for the recovery of any amount or amounts above assigned and to give receipts for same, or any part
thereof, in my name and I hereby release and discharge my said employers and each of them from all liability to me for or on account of any or all monies paid in
accordance with the terms hereof. Nothing herein shall prevent the Credit Union from exercising any other right of recovery available in law of any amount lawfully
owing to the Credit Union in excess of the amounts assigned above. THIS ASSIGNMENT OF WAGES is irrevocable by the Assignor. and shall remain a valid and
enforceable security in respect of all debts owing by the Assignor to the Credit Union until delivery of notice in writing signed by the Credit Union to the contrary.
