The 2012 Mission-Critical Innovation Awards, presented by HP and Intel®, recognizes leaders in IT for
their groundbreaking work developing solutions for mission-critical computing. Please carefully read the
Official Awards Rules at the end of this document and complete all fields before submitting your entry. To
be eligible for the Awards, you must check the box at the end of this document indicating that you
have read and agree to the Award Rules. Do not insert graphics or other files into this document. All
text must be in 10-point Arial font and adhere to the word counts specified for each answer. Please read

Official Awards Rules
BY ENTERING THE 2012 MISSION-CRITICAL INNOVATION AWARDS, PRESENTED BY HP AND
INTEL, PARTICIPANTS AGREE TO BE UNCONDITIONALLY BOUND BY THESE OFFICIAL AWARD
RULES.
1. THE 2012 MISSION-CRITICAL INNOVATION AWARDS, PRESENTED BY HP AND INTEL, IS VOID
WHERE PROHIBITED OR RESTRICTED BY LAW.
2. Sponsors
The 2012 Mission-Critical Innovation Awards (the “Awards” or “Awards program”) is co-sponsored by
Hewlett-Packard (HP), 3000 Hanover Street, Palo Alto, CA 94304-1185, USA and Intel, 2200 Mission
College Blvd, Santa Clara, CA 95054, USA.
3. Overview
The object of the Awards is to submit the most compelling and most innovative mission-critical computing
solution in one of four categories as defined in section 6: (a) Mission-Critical Data; (b) Data Center
Modernization; (c) Humanitarian/Environmental Impact; or (d) Best New Application.
4. Eligibility Requirements
NO PURCHASE NECESSARY.
The Awards are open to any solution using at least one Intel Itanium processor (or Xeon processor in the
Data Center Modernization category), which has been deployed in an operational configuration prior to
the time this Solution is entered into the Awards (“Solution”). There is no limit on the number of Solutions
that may be entered by a person or other entity, but any particular Solution may only be entered once per
annum.
HP and Intel hold as their right the option to require confirmation of results submitted as part of the
verification process, and hold the right to disregard any submission at their sole discretion.
5. Entry Procedure
The due date for entries is October 20, 2012. Entries that are late or incomplete for any reason may be
discarded and may not be judged. Note: If you are in the process of filling in a submission, and feel you
may miss deadline, please contact us: mcia@hp.com.
To enter, complete and submit this official entry form; the original can be found on the Awards website at
http://www.hp.com/go/mcia. Entries may be submitted online or sent via email to mcia@hp.com. If
necessary, Awards personnel or judges may request additional information regarding a Solution,
including but not limited to additional documentation, an interview of Solution developers, or a
demonstration. Entrants must promptly comply with any requests for additional information as a condition
of continued eligibility. HP and Intel offer entrants the option to include a short-form (2-3 minute) video
with their entry to better explain their Solution; instructions for submitting video are included on the
Awards website and any videos received become property of HP and Intel with no restrictions on use.
Entry forms and other Award submissions shall be submitted in English only. If you are struggling with the

materials become the sole property of HP and Intel and will not be returned. HP and Intel may, at their
discretion, use text and videos received as part of Award submissions in their own promotions before,
during, and after the Awards program.
6. Judging Criteria
A Solution may be submitted in one of four categories: (a) Mission-Critical Data; (b) Data Center
Modernization; (c) Best New Application; or (d) Humanitarian/Environmental Impact. HP and Intel may, at
their discretion, move an entry from one category to another. Winners will be chosen by a panel of Award
judges based on the following criteria:
(a) Mission-Critical Data: This category recognizes the success organizations have delivered by
implementing a database, data warehousing, or line-of-business solution that takes advantage of
the dependability and performance of Itanium-based platforms while producing impactful results.
(i) Results produced by solution
(ii) Cost/benefit of solution
(iii) Difficulty of problem addressed by solution
(iv) Originality of solution
(b) Data Center Modernization: This category highlights the success of projects that are modernizing
mission-critical computing environments with: migrations to Itanium-based systems, and/or
effectively introducing Xeon-based servers for the most demanding x86 workloads, and/or
utilising blade servers for mission-critical computing.
(i) Results produced by solution
(ii) Cost/benefit of solution
(iii) Difficulty of problem addressed by solution
(iv) Originality of solution
(c) Best New Application: This category highlights the software solutions being developed that best
fulfill an industry need while effectively harnessing the capabilities inherent in Itanium-based
systems.

(d) Humanitarian/Environmental Impact: This category recognizes an organization whose use of
Itanium-based systems is making a considerable, positive impact upon society or the
environment.

All determinations by the judges are final.
7. Prizes
Finalists and winners of the Awards will receive recognition and publicity for their achievements. All
federal, state, and local taxes and any other expenses associated with the prize are the sole responsibility
of the winner. No prizes will be awarded to any participant who is not in compliance with these Official
Awards Rules, or with applicable law.
8. Notification of Winners
Finalists will be notified of their status by email in early November. All category winners will be announced
December 3, 2012 at the Awards event in Frankfurt, Germany. All finalists are strongly encouraged to
attend the Awards celebration event. However, attendance status will not affect the judging of
submissions in any way.
9. Internet
If in HP and Intel’s sole judgment there is any suspected or actual evidence of tampering with any portion
of the Awards, or if computer viruses, bugs, unauthorised intervention, fraud, or technical difficulties or
failures compromise or corrupt or affect the administration, integrity, security, fairness, or proper operation
of the Awards, HP and Intel reserve the right, in their sole discretion, to disqualify any individual who
tampers with the entry process and/or to void any entries submitted fraudulently or not in accordance with
these Official Awards Rules.
CAUTION: ANY ATTEMPT BY A PARTICIPANT TO DELIBERATELY DAMAGE THE WEBSITE OR
UNDERMINE THE OPERATION OF THE AWARDS MAY BE IN VIOLATION OF CRIMINAL AND CIVIL
LAWS, AND, IN SUCH EVENT, HP AND INTEL RESERVE THE RIGHT TO PURSUE THEIR
REMEDIES AND DAMAGES (INCLUDING COSTS AND ATTORNEYS’ FEES) TO THE FULLEST
EXTENT OF THE LAW.
10. Permission to Use Information about Solutions, Finalists, and Winners
As a condition of entering into this Awards program, participants agree that HP and Intel may use, for all
purposes and without compensation, all information or other submission of any kind submitted in the
Awards.
Winning participants agree that as a condition of accepting any prize, they will cooperate with HP and
Intel in providing information regarding themselves and the winning Solution, including without limitation:
the winner’s name and likeness(es), case studies, articles, press releases, performance data, videos, and
photos of their Solutions (if applicable). All such information may be used by HP and Intel for all purposes
and without compensation.
Participants will not be required to disclose any personal financial information to enter the Awards.
11. Conditions
As a condition of entering into this Awards program, participants agree to the following:
(v)
Participants agree to be bound by these Official Awards Rules and HP and Intel’s
determination of the winners, which shall be final.
(vi) Participants agree to release and hold harmless HP and Intel and their employees, officers,
directors, members, assigns, attorneys, representatives, and agents, from all liability arising
from or relating to the Awards or any prize won, including all liability arising from
infringement of the intellectual property rights of any party. Participants agree to waive all
rights to claim exemplary, punitive, incidental, or consequential damages, or any other
damages other than for actual out-of-pocket expenses, and that all causes of action arising
out of or connection with this Awards program, or any prizes awarded, shall be resolved
individually, without resort to any form of class action.
(vii) Participants agree that this Awards program is governed by the laws of the State of
Oregon, United States, and that any dispute arising from this Award will be governed by,
and construed in accordance with the laws of the State of Oregon, without reference to
conflict of law principles, and the participants consent to personal jurisdiction in the state
and federal courts of the State of Oregon and agree that such courts have exclusive
jurisdiction over all such disputes.
(viii) HP and Intel reserve the right to cancel the Awards at any time without obligation or prior
notice.
