FLORIDA VIRTUAL SCHOOL–STUDENT AGREEMENT
I.D. #____________________ Name: _________________________________________ Gr._____
Course: ______________________________________________________ Start Date______________
Under the Digital Learning Act of 2011 all incoming 9th grade students are required to complete
one high school level virtual course prior to graduation. To register for any course through
Florida Virtual School (FLVS), please visit their website: www.flvs.net.
Florida Virtual School sets high expectations for the success of its students. They are committed to
meeting the needs of all students and will work tirelessly to facilitate student success. To maintain
instructional momentum, students are required to submit assignments in their course on a weekly basis
and to maintain regular contact with their instructors.
 Any student who does not make progress in the course for a period of 14 days and has not made
prior arrangements with his/her instructor will be referred to the Student Success Center in
attempt to reconnect the student with the course instructor.
 The Student Success Center will attempt to reconnect the student and parent with the course
instructor to create a Success Plan for the student.
 Failure to respond to the Student Success Center's attempts to contact the student, or failure to
adhere to the Success Plan created by the student and family may result in Administrative
Removal from the course with an F grade.
Students are expected to agree to the following as part of the registration process.
I acknowledge that during the first 14 days of being activated into my FLVS course I may drop the course
without penalty. I understand that for each online course there are a minimum number of assignments
that must be completed each week. Failure to submit the minimum number of assignments on a weekly
basis will result in my removal from the course and may result in a failing grade being assigned to my
academic transcript. If I drop the course after completing 50% of the class requirements and fail to take
the final exam, Florida Virtual School will issue an "F" for my final grade.
If I decide to drop a FLVS course, I must see my high school counselor to obtain approval. If I do not
complete the course(s) I have taken, and FLVS drops me from the courses with a WF or if I
drop a course after the grace period and receive a WF, it will count as “F” on my GPA and on
my official transcript

How do I sign up?
Go to flvs.net and follow the easy step-by-step directions.
Why do I sign up?
Students who are graduating in 2015 and beyond are required to take at least
one online class, a minimum of .5 credits, to graduate. You must take both parts
of a 1 credit class to meet the online requirement.
When do I sign up?
The required online class can be taken anytime following 8th grade graduation
and prior to senior graduation.
A few tips
 When you sign up on Florida Virtual School be sure to mark you are
attending a public/charter school, your school is PATHS, and put your
counselor’s name.

Mr. Merrill - last names beginning with A-L

Ms. Smith – last names M-Z.

We generally do NOT recommend taking required academic classes on
line.

You may take HOPE pt.1 and pt. 2 online. This HOPE class requires a log
of activities, projects, CPR training, etc. making it more challenging than
taking HOPE at PATHS. If a foreign language does not fit into your
schedule, or you would like to take a different foreign language, you may
sign up on line. Be sure to include both part 1 and part 2 for each. Florida
universities require two years of the same foreign language.

Go t o www.flvs.net/electives to learn what they have to offer. There are a
lot of interesting electives that will meet the online requirement. You may
be interested in a new elective they are offering; Social Media (.5) credit.

Students who enter 9th grade starting in 2013 and beyond are NOT able to
use Driver’s Ed. for their on line graduation credit, yet can still take the
class. Current sophomores and juniors may take Driver’s Ed. to fulfill the
online requirement.

Please fill out the Student Agreement, on the front side, and return to
PATHS guidance only after you have signed up for a class.
