Health At Home is a Medicare certified, licensed and accredited home health agency specializing in
providing skilled and private duty services to patients who reside in private senior communities and
the general population.
The Director of Operations is responsible for directing all aspects and overall financial health of the
agency. The role requires a unique balance between operational expertise and business development
acumen.
2012 – 2012
Administrator [Gateway Home Health and Hospice] – Clarksville, Tennessee
Gateway Home Health and Hospice operates an independently licensed, Medicare Certified and
CHAP accredited home health agency and hospice agency directed by a single Administrator.
As Administrator, the responsibilities included oversight and direction of both agencies, with specific
emphasis on, regulatory and corporate compliance, operational excellence, business growth, and
financial results.
2010 – 2011
Executive Director/Adm. [Matrix Medical Home Care, LLC] - Plant City, Florida.
Matrix Medical Home Care is a Medicare certified, licensed and accredited home health agency
providing skilled nursing, physical, speech and occupational therapies, medical social services and
home health aides to patients in their homes.
The position required creating a start-up home health agency to meet or exceed the State of Florida,
Medicare, and CHAP standards and guidelines. The responsibilities included developing and
overseeing all aspects of the agency i.e., policies, procedures, Information systems, regulatory
compliance systems, sales and marketing, and human resources for appropriate service delivery.

Owner/Administrator [National Nursing Pool, Inc.] - Oakland Park, Florida.
National Nursing is certified home health agency that provides professional and para-professional
services including skilled nursing, therapy, medical social services, home health aide, live-in,
companion, and CNA services to patients.
The position reported to the Board of Directors. The responsibilities were to direct the day-to-day
operations within state and federal guidelines and supervise an administrative staff of eight (8)
employees and clinical staff of one hundred (100) employees. Additionally, to ensure the viability and
financial health of the home health agency by setting company goals and objectives and implement
strategies to meet those goals and objectives.
1998 - 2006
Owner/Administrator [Consummate Rehabilitation Services, Inc.] - Hollywood, Florida.
Consummate Rehabilitation Services provided medical rehabilitation services including physical,
occupational and speech therapies for the treatment of patients with acute injuries or conditions in
an outpatient setting.
As an owner operator, the responsibilities included general oversight of the business and management
of the day-to-day operations. The company was developed from a business plan in 1998. Began
operations, obtained Medicare certification and 30 managed care contracts in by 1999, and grew to a
five (5) outpatient facilities operation by 2005.

 Developed turnkey facilities for clients within the areas of home health, outpatient rehabilitation, durable medical
equipment and sleep diagnostic testing centers.
 Secured national, regional, and local managed care contracts for a variety of services and clients.
 Prepared various providers for and obtained AHCA licensing, Medicare Part A certification, Medicare Part B
certification, Medicaid certification, and Accreditation from CHAP and ACHC.
 Developed regional and local ancillary provider networks to meet the needs of specific payers.

 Successfully increased revenue by ninety (90%) within an eighteen (18) month period with self-owned business.
