Professional Development Activity/Plan Template
Use this form to create a facilitator guide for a professional development activity. It is patterned after the format
used with the Build and Deliver Modules. This will primarily be of value to you for an opportunity with your
identified Potential Educator Audience, so it can be aimed at a very small group or a larger one, depending on
your needs or context. However, keep in mind that this should be clear and complete enough that another
facilitator can easily use it in another educational setting. We will post this on our website so others can benefit,
both locally and across the TPS consortium.

Preparation
Replace text Detail what facilitator (including yourself!) needs to prepare PRIOR to beginning
the activity with the participants.
For example:

 Remember to include model strategies to be used with students.

 Write procedures as concisely as possible using clear, direct language.
Replace text How will know your audience of educators have successfully reached the
objectives? Make sure the assessment/reflection is clear and realistic for adult learners. If a
