FORMAT & INSTRUCTIONS
Deadline: Please submit your completed electronic reports to WisTAF via email with attachments to
rmurray@wistaf.org no later than 4:00 p.m. on Friday, July 31, 2009.
Combined Report Instructions
In an effort to make reporting more efficient for grantees, we have combined the 2009 mid-year report for
IOLTA/PILSF grants with the 2009 annual report for state appropriation grants. Please complete the following
report sections for the type of grants you received:
2009 IOLTA/PILSF Grant Mid-Year Report – Complete this section if you received 2009 IOLTA Category
I, 2009 IOLTA Category II and/or 2009 PILSF grants. The time period for which you will be reporting is
January 1, 2009 through June 30, 2009.

 Note: The 2008-09 State Appropriations Outcome Report requests information on the number of
clients served; the number of cases open at some point during 7/1/08–6/30/09; and the monetary
value received/recovered on behalf of clients for those matters in which state appropriation funding
was used. If you are unable to track outcomes separately by funding source, please calculate the
overall percentage of income your state appropriation grant accounted for during the year and apply
that percentage to total clients served; total cases open; and the monetary value received/recovered.
For example, if you received a $10,000 state appropriation grant accounting for 40% of your total
income, use .4 as the multiplier to come up with your outcome estimates.
