after-school program to submit a formal application for our upcoming Leadership Training ACADEMY,
which includes eight (8) - 3 ½-hour - sessions held from April through June each year. Completing the
application requires approximately 1 ½ to 2 hours. To be considered, we must receive one copy of the
following information:

Academy Application (signed by Executive Director)
List of Board of Directors (include title & company affiliation)
Current Agency and/or After-School Program Budget
Financial Statements from most recently completed fiscal year
Copy of 501(c)(3) tax-exemption letter
Any additional supporting information to help us learn more about your after-school program
and the services you offer (please do not send videos)
Completed applications must be postmarked no later than January 31. We are unable to
consider incomplete applications and any applications received via fax or e-mail.

Site visits will be conducted with all after-school programs that are selected as finalists. Programs selected to
participate will be notified by March 20.
Please see our Program Calendar (available at www.cityscholars.org, go to the What We Offer or How We
Help page) for a list of upcoming ACADEMY session dates and times. Dates for the following year’s
ACADEMY are posted each year in November/December.
Each after-school program selected for the Leadership Training ACADEMY is asked to designate as
many as three (3) core management staff members to participate as ACADEMY Fellows with all eight (8)
sessions of the ACADEMY program. While participation of an agency’s Executive Director is not required, it is
strongly preferred to help ensure full support for the implementation of new ideas, concepts and best practices
shared throughout the ACADEMY program.
To formally graduate from the Leadership Training ACADEMY and be recognized at our gala
Anniversary Celebration held each year in September, ACADEMY Fellows must actively participate with at
least seven (7) of the eight (8) sessions.
Please note that if your after-school program is accepted to participate with our Leadership Training
ACADEMY program, we may include a brief profile of your organization and its services on our website and
throughout our communications materials. By submitting this application, you are agreeing on behalf of your
after-school program to allow City Scholars Foundation to publish this information.
