Please submit this form to the Graduate Dean’s Office by the drop/add deadline of the student’s last semester of
undergraduate studies. In order to properly code your student for the graduate portion of the degree, please include the
student’s most recent undergraduate audit. Note that only three graduate courses will be double-counted toward both
undergraduate and graduate requirements. Any additional graduate course taken while an undergraduate will count
toward the undergraduate requirements only and be listed on the student’s undergraduate transcript, but it will not be
allowed to “transfer” or count toward his or her graduate credits.

Please note: The student will need to have earned a grade of “B” or better in any course listed on this sheet for which the
course carries a normal (N) grade mode. For courses carrying a Satisfactory/Unsatisfactory (S) grade mode, the student will
need to have earned an “S” grade. Normal graded courses for which the student elected to pursue under “S” grade
mode cannot be double counted. The student’s graduate status becomes official only after the student forwards his/her
final undergraduate transcript to the Dean’s Office documenting the degree.
