TITLE:
Geriatric Care Specialist, Special Populations
REPORTS TO:
Director, Geriatric Care Management and Caregiver Support
DEPARTMENT:

JOB SUMMARY:
Under direction, provides Geriatric Care Management, telephone
consultation and referral services to older adults and family
caregivers to assist with a variety of elder care issues including
future planning and crisis situations. Provides counsel and
support to caregivers to maintain their wellbeing and optimal
function.
FLSA CLASSIFICATION: Exempt
RESPONSIBILITIES:
A. Performs responsibilities of Geriatric Care Management and Consultation and Referral in
compliance with Sheltering Arms Senior Services standards for service delivery, with
priority/emphasis on special groups: family caregivers, Veterans, health care system
referrals, and SASS Day Center families.
1. Provides telephone and/or office consultation with clients according to their specific
elder care concern.
2. Provides comprehensive biopsychosocial assessments in the home of the caregiver or
older adult.
3. Provides current referral information to clients both over the phone and in written
report form. Visits sites, makes and records observations to enhance referral
information.
4. Maintains all client, resource, and provider records. Documents completely and
timely all client and client-related contacts.
5. Tracks and reports outputs and other data as needed by the Agency for accounting,
billing, and evaluation.
6. Participates in development of and delivers presentations for Corporate Caregiver
Initiative; provides community education through presentations, fairs, etc.
7. Leads caregiver support groups, including Powerful Tools for Caregivers, maintains
documentation and reports re: outputs and outcomes.
8. Provides back-up to other department services and to Access and Information, as
necessary.
B. Maintains an effective working relationship with other agency personnel to achieve
maximum team approach to service delivery and support morale.

C. Attends agency conferences and meetings; participates in in-service trainings and seminars.
Maintains certification or licensure.
D. Performs other duties as assigned.
QUALIFICATIONS:
Required:
1) Master’s degree in human services field, and experience in case management, geriatric care
management and/or consultation
2) Eligible for certification as Geriatric Care Manager (i.e., CMC per NAPGCM)
3) Knowledge of community services for the elderly.
4) Excellent written and verbal communication skills, including presentations.
5) Excellent computer skills.
6) Bilingual (Spanish) skills a plus.
Preferred
1) LCSW or other clinical practice license and experience.
2) Experience in health care setting, working with insurance benefits for older adults.
SPECIFICATIONS:
Physical demands: Work is sedentary. May require some light lifting and bending.
Work environment: Work is performed in a typical office setting.
