Instructions for completing the application form for the School Human
Movement Studies Alumnus of the Year Awards




Nominations may only be submitted by a third party (internal or external) – not by selfnomination
Nominees must be HMS Alumni.
Only complete the sections relevant to the application.
In addition to the application form, please ensure that a current copy of the nominee’s
Curriculum Vitae is attached. Other supporting evidence: awards, media clippings,
electronic media footage may also be included.
Nomination deadline is 5 pm (Brisbane time) Tuesday 15th October 2013

Awards Eligibility:
Young Alumnus of the Year
Alumnus of the Year
Nominees for this award must be 34 years
of age or under as of 15 October 2013 and
be a graduate that holds a Bachelor,
Masters or PhD from The School of Human
Movement Studies at the University of
Queensland. Additionally nominees will be
assessed on their demonstrated
achievements in one or more of the
following areas:
 Exceptional success in their profession
or industry
 Significant contribution to their
profession or industry
 Significant contribution to the wider
community
Nominees for this award must be a
graduate that holds a Bachelor, Masters or
PhD from The School of Human Movement
Studies at the University of Queensland.
Additionally nominees will be assessed on
their demonstrated achievements in one or
more of the following areas:
 Sustained exceptional success in their
profession or industry
 Sustained significant contribution to
their profession or industry
 Significant contribution to the wider
community
Value based decisions will be made by the
judging panel based on the applicant's
ability to demonstrate the achievements
they have gained across these criterion;
and qualities such as leadership, integrity,
innovation and dedication.
