Description
The purpose of the INNOVATION and CREATIVITY award is to recognize and reward
employees who introduce ideas, services or practices in new and creative ways. These
individuals contribute innovative solutions to their organization or make major
improvements to agency operations or services provided.
Traits of Ideal Candidates
 Introduce ideas that increase revenue, reduce costs or improve service
 Enhance efficiency, accuracy or productivity in innovative ways
 Contribute creative ideas to address issues within the scope of their job and organization
 Introduce changes to simplify or improve existing processes
 Challenge existing assumptions and think outside the box
 Take calculated risks in pursuit of continuous improvement
 Work with others to promote, implement and adjust solutions to changing situations
How to Submit
 Nomination forms must be submitted by email (awards@unitedwaypeel.org) no later
than Friday, May 22, 2015 at 5 p.m.
 Remember: Winners are determined solely on this nomination form. Please be as
specific as possible when describing projects/accomplishments
How the Award Works
 Employees may nominate their peers but downward nominations are not accepted
 No self-nomination or nomination of family members
 All information will be verified by the nominee’s supervisor
 Achievements must occur during the time period from January 2014 to December 2014
