Checklist of Application Materials for
Prague, Czech Republic (CET)
STEP 1:
Complete the IAP on-line application at http://www.studyabroad.wisc.edu/applications/apply.html
STEP 2:
Gather and complete the following application materials:

CET Application (SUBMIT ONLINE)
Complete the CET application online at https://cet.studioabroad.com/index.cfm?FuseAction=Security.Login You will submit the following
materials online directly to CET as part of the CET online application:



Central European Studies or Jewish Studies Application Form
Course Request Form
CET Letter of Recommendation #1
On your “Program Application” page, click on “Request Electronic Recommendation” under the Recommendations heading.
You’ll need to complete your professor or teaching assistant’s information and any courses you took with them. Click on Save
to have an electronic recommendation request sent to the instructor.

CET Letter of Recommendation #2
On your “Program Application” page, click on “Request Electronic Recommendation” under the Recommendations heading.
You’ll need to complete your professor or teaching assistant’s information and any courses you took with them. Click on Save
to have an electronic recommendation request sent to the instructor.

CET Letter of Recommendation #3 (Czech language evaluation, if applicable)
On your “Program Application” page, click on “Request Electronic Recommendation” under the Recommendations heading.
You’ll need to complete your professor or teaching assistant’s information and any courses you took with them. Click on Save
to have an electronic recommendation request sent to the instructor. If you have no experience with the Czech language, you
do not need to submit this 3rd recommendation.

CET Application Fee: $50
A non-refundable application fee of $50 is required with the completed application. This fee will be applied to the cost of the program for
accepted students. Payment can be in the form of a check or money order to 'CET Academic Programs'. Please turn in the application fee
with your other materials and IAP will forward it on to CET.

IAP Statement of Responsibility + one photocopy

Official transcript(s) + one photocopy
Official transcript(s) and copy from UW-Madison and any other college/university you have attended. UW-Madison transcripts may be
obtained at the Registrar Office. Copies of non-UW transcripts may be obtained in 70 Bascom Hall.

Passport-size Photos – 6 Photos
The passport-size photo must be 2x2 inches, original color photo on photo paper, face forward on light background and name printed on
back—scanned photos or photos cut to size are not acceptable.

Czech Student Visa Materials (semester only)
Under the “Material Submissions” box on your on-line application, you will find detailed instructions on what materials you need to prepare
for your Czech student visa application. Due to the large amount of time required by the Czech government for visa processing, these
materials are due at the same time as your application.
STEP 3:
Submit Supplemental application materials listed in Step 2, including this checklist page, by the
application deadline date to:
